摘  要:目的分析全身亚低温序贯高压氧治疗新生儿缺氧缺血性脑病(HIE)疗效及对血清神经元特异性烯醇化酶(NSE)的影响。方法选取2018年9月—2020年3月在厦门市妇幼保健院确诊并治疗的72例HIE患儿为研究对象,依据不同治疗方式分组,对照组全身亚低温,研究组联合序贯使用高压氧。观察比较两组治疗前、治疗后的行为神经测定(NBNA)量化评分水平及差异;两组治疗前、治疗后的NSE水平及差异;两组的疗效指标:症状消失、恢复自主吮吸能力及使用呼吸机时间。结果研究组治疗后不同时间点下的NBNA量化评分水平显著高于对照组(P<0.05)。研究组治疗后不同时间点下的NSE水平显著低于对照组(P<0.05)。研究组症状消失、恢复自主吮吸能力及使用呼吸机时间均显著短于对照组(均P<0.05)。结论给予HIE患儿序贯使用全身亚低温及高压氧进行治疗,可显著缩短HIE患儿的康复时间,提高疗效,降低NSE水平。Objective To analyze the effect of systemic mild hypothermia and sequential hyperbaric oxygen therapy on neonatal hypoxic ischemic encephalopathy(HIE)and its effect on serum neuron specific enolase(NSE).Methods 72 children with HIE diagnosed and treated in Xiamen maternal and child health hospital from September 2018 to March 2020 were selected as the research objects.They were grouped according to different treatment methods.The control group used hyperbaric oxygen in whole body and the study group used hyperbaric oxygen in sequence.The quantitative scores of behavioral neurometry(NBNA)before and after treatment were observed and compared between the two groups;NSE levels and differences between the two groups before and after treatment;The efficacy indexes of the two groups:disappearance of symptoms,recovery of self sucking ability and time of using ventilator.Results The quantitative score of NBNA in the study group was significantly higher than that in the control group at different time points after treatment(P<0.05).The NSE level in the study group at different time points after treatment was significantly lower than that in the control group(P<0.05).The time of symptom disappearance,recovery of self sucking ability and use of ventilator in the study group were significantly shorter than those in the control group(P<0.05).Conclusion The sequential use of systemic mild hypothermia and hyperbaric oxygen in the treatment of HIE children can significantly shorten the rehabilitation time of HIE children,improve the curative effect and reduce the level of NSE.
