贺氏火针治疗脑卒中恢复期痉挛疗效观察

摘　　要：目的观察贺氏火针治疗脑卒中恢复期痉挛患者的临床疗效。方法选择2019年1月—2020年12月首都医科大学附属北京中医医院针灸科收治的脑卒中恢复期痉挛患者120例,采用随机、对照、单盲(评价者盲)方法将患者分为治疗组和对照组,每组60例。治疗组给予贺氏火针治疗,对照组给予常规针刺治疗,2组均治疗4周。观察比较2组治疗前及治疗2周、4周和12周后改良Ashworth量表(MAS)评分、简氏Fugl-Meyer评定量表(FMA)评分、Barthel指数、脑卒中专用生活质量量表(SS-QOL)评分及治疗12周后肌张力情况。结果2组治疗4周后MAS评分、Barthel指数评分、SS-QOL评分均较治疗前明显增高(P均<0.05),FMA评分与治疗前比较差异无统计学意义(P均>0.05)。2组各项评分比较差异均无统计学意义(P均>0.05);2组治疗12周后MAS评分、FMA评分、Barthel指数评分、SS-QOL评分均较治疗前明显增高(P均<0.05),且治疗组各项评分均明显高于同期对照组(P均<0.05)。治疗12周后,治疗组肌张力明显增高(Ashworth评分≥3分)患者比例为17.9%(10/56),对照组为35.2%(19/54),治疗组明显低于对照组(P<0.05)。结论贺氏火针可以降低脑卒中恢复期患者肢体痉挛程度,促进运动功能恢复,有效改善患者的生活质量。Objective It is to observe the clinical effect of He’s fire acupuncture in patients with stroke convalescent spam.Methods A total of 120 patients with stroke convalescent spasm who were admitted to the Department of Acupuncture and Moxibustion,Beijing Hospital of Traditional Chinese Medicine Affiliated to Capital Medical University from January 2019 to December 2020 were selected,and the patients were randomly divided into two groups by the method of randomization,control,and single-blind(evaluator-blind),each group had 60 cases.The treatment group was treated with He's fire acupuncture,and the control group was treated with conventional acupuncture.Both groups were treated for 4 weeks.The scores of modified Ashworth scale(MAS),Jane's Fugl-Meyer Assessment(FMA),Barthel index,and stroke-specific quality of life(SS-QOL)before treatment and at 2 weeks,4 weeks and 12 weeks after treatment,and the changes in muscle tone after 12 weeks of treatment were observed and compared between the two groups.Results After 4 weeks of treatment,the MAS score,Barthel index score and SS-QOL score in both groups were significantly improved compared with those before treatment(all P<0.05),there was no significant difference in FMA score compared with before treatment(P>0.05).There was no significant difference in these indexes between the two groups(P>0.05).After 12 weeks of treatment,the MAS score,FMA score,Barthel index score,and SS-QOL score in both groups were significantly improved compared with those before treatment(all P<0.05),and the improvement of each score in the treatment group was significantly higher than that in the control group(P<0.05).After 12 weeks of treatment,the proportion of patients with significantly increased muscle tension(ashworth score≥3)in the treatment group was 17.9%(10/56),which was significantly lower than that 35.2%(19/54)in the control group(P<0.05).Conclusion He’s fire acupuncture can reduce the degree of limb spasm in convalescent stroke patients,promote the recovery of motor function,and ef
