正畸治疗中下颌骨旋转对上呼吸道容积变化的影响

摘　　要：目的:探讨成人骨性Ⅱ类正畸治疗中,下颌骨旋转对上呼吸道容积的影响。方法:收集骨性Ⅱ类错[牙合]患者正畸治疗前后的CBCT数据,根据下颌平面旋转,将样本分为顺时针旋转组(CR组20例)与逆时针旋转组(CCR组19例)。通过Dolphon 11.95软件,对不同组上呼吸道进行重组、测量与分析。结果:CCR组下颌平面减小1°,CR组下颌平面则增加1.1°。治疗后,CR组的鼻咽容积增加0.69 cm^(3)(P<0.05)、CCR组增加0.45 cm^(3)(P<0.05),但两组间的变化量无统计学差异(P>0.05)。腭咽容积、舌咽容积、总体积及最小横断面积两组间均未见统计学差异。结论:正畸治疗成人骨性Ⅱ类错[牙合]后,鼻咽容积有所增加;但下颌骨旋转对上呼吸道未造成明显影响。Objective:To explore the influence of mandibular rotation on the morphology of upper airway during orthodontic treatment in adults with skeletal Class II malocclusion.Methods:Retrospective analysis was performed on pre-and post-treatment CBCT data of the adults with skeletal Class II malocclusion.The subjects were divided into 2 groups according to the rotation direction of the mandibular plane:clockwise rotation(CR,n=20)and counterclockwise rotation(CCR,n=19).The upper airway was reconstructed,measured and analyzed by Dolphin 11.95 software.Results:The mandibular plane decreased by 1°in CCR group,but increased by 1.1°in CR group.After treatment,the nasopharyngeal volume was increased by 0.69 cm 3 in CR group(P<0.05),and 0.45 cm 3 in CCR group(P<0.05)(between groups,P>0.05).There also were no statistical differences in velopharyngeal volume,glossopharyngeal volume,total volume,and minimum cross-section between the 2 groups.Conclusion:The volume of nasopharynx of adult skeletal Class II patients increased after orthodontic treatment,but the upper airway was not significantly affected by the rotation of mandible.
