血清CysC、Lp-PLA2水平对短暂性脑缺血发作患者硫酸氢氯吡格雷联合瑞舒伐他汀治疗效果的影响

摘　　要：目的研究血清胱抑素(Cys)C、脂蛋白磷脂酶(Lp-PL)A2水平对短暂性脑缺血发作(TIA)患者硫酸氢氯吡格雷片联合瑞舒伐他汀治疗效果的影响。方法选取宣城市人民医院2018年8月至2020年8月收治的106例TIA患者作为研究对象,患者均在本院接受硫酸氢氯吡格雷片联合瑞舒伐他汀治疗,治疗2周时,评估2组治疗效果,分为治疗无效组(n=28)、治疗有效组(n=78)。入院当天检测患者血清CysC、Lp-PLA2水平,询问并记录2组患者一般资料,分析血清CysC、Lp-PLA2水平与TIA患者硫酸氢氯吡格雷片联合瑞舒伐他汀治疗效果的关系。结果治疗无效组血清CysC、Lp-PLA2水平均高于治疗有效组,差异有统计学意义(P<0.05),经Logistics回归分析显示,血清CysC、Lp-PLA2过表达与TIA患者硫酸氢氯吡格雷片联合瑞舒伐他汀治疗无效有关(P<0.05)。绘制受试者工作特征曲线图(ROC)显示,血清CysC、Lp-PLA2单独及联合预测TIA患者硫酸氢氯吡格雷片联合瑞舒伐他汀治疗无效的曲线下面积(AUC)分别为0.780、0.815、0.868,均有一定预测价值。结论血清CysC、Lp-PLA2过表达与TIA患者硫酸氢氯吡格雷片联合瑞舒伐他汀治疗无效有关,入院时检测血清CysC、Lp-PLA2水平可预测TIA患者治疗无效风险。Objective To investigate the effects of serum levels of cystatin(Cys)C and lipoprotein phospholipase(Lp-PL)A2 on the therapeutic effects of clopidogrel bisulfate tablets combined with rosuvastatin on transient ischemic attack(TIA).Methods A total of 106 patients with TIA who were treated in our hospital from August 2018 to August 2020 were enrolled in the study,who were treated by clopidogrel bisulfate tablets combined with rosuvastatin.After 2-week treatment,the therapeutic effects were evaluated,then the patients were divided into ineffective treatment group and effective treatment group.The serum levels of CysC and Lp-PLA2 were detected on the day of admission,and the general data of patients were recorded in both groups,and the correlation between the serum levels of CysC,Lp-PLA2 and the therapeutic effects was analyzed.Results The serum levels of CysC and Lp-PLA2 in ineffective treatment group were significantly higher than those in effective treatment group(P<0.05).Logistic regression analysis showed that overexpression of serum CysC and Lp-PLA2 was related to the ineffective treatment of clopidogrel bisulfate tablets combined with rosuvastatin in patients with TIA(P<0.05).The receiver operating characteristic curve(ROC)was drawn and showed that area under the curve(AUC)of serum CysC and Lp-PLA2 alone and combination to predict the ineffective treatment were 0.780,0.815 and 0.868,respectively,which had certain predictive value.Conclusion The overexpression of serum CysC and Lp-PLA2 is related to the ineffective treatment of clopidogrel bisulfate tablets combined with rosuvastatin in patients with TIA,and to detect the serum levels of CysC and Lp-PLA2 on admission can predict the risk of ineffective treatment in patients with TIA.
