经直肠超声造影和彩色多普勒超声用于直肠癌与直肠间质瘤鉴别诊断的价值对比分析

摘　　要：目的:比较经直肠超声造影(CEUS)和彩色多普勒超声(CDU)用于直肠癌与直肠间质瘤鉴别诊断的价值。方法:随机抽取2019年9月至2020年9月于我院接受手术治疗且术前行CEUS和CDU检查的直肠癌或直肠间质瘤患者各40例,比较直肠癌与直肠间质瘤患者CEUS、CDU检查参数的差异,以及两种检查方法对直肠癌和直肠间质瘤的诊断准确率。结果:1)CDU检查参数:直肠癌组患者病灶包绕肠腔率、病灶内部回声均匀率、阻力指数、血流峰值流速均显著高于直肠间质瘤组,P<0.05。2)CEUS检查参数:直肠癌组患者峰值强度(Imax)明显高于直肠间质瘤组,而上升时间(RT)、达峰时间(TTP)、平均渡越时间(mTT)明显低于直肠间质瘤组,P<0.05。3)诊断准确率:CEUS诊断直肠癌及直肠间质瘤的准确率(96.25%)明显高于CDU(81.25%),P<0.05。结论:CEUS和CDU用于鉴别诊断直肠癌与直肠间质瘤均具有一定价值,但两种检查方式相比,CEUS效果更好。Objective To compare the value of contrast-enhanced ultrasound(CEUS)and color Dopple ultrasound(CDU)in differentially diagnosing rectal cancer from rectal stromal. Methods Randomly enrolled rectal cancer patients(40cases)and rectal stromal patients(40cases)who all received surgery(2019-09-2020-09)in author’s hospital,and preoperative CEUS and CDU inspection;then,compared both group’patients’ difference between CEUS parameter and CDU one,as well as diagnostic accuracy rate of both inspections on rectal cancer and rectal stromal. Results 1)CDU parameters inspected:in the rate of bowel cavity encysted,echophony smooth rate,resistance index,and flow peak velocity the patients with rectal cancer were all significantly higher than the patients with rectal stromal(P<0.05);2)CEUS parameters inspected:in Imax.the patients with rectal cancer were significantly higher than the patients with rectal stromal(P<0.05),and its RT,TTP,and mTT lower than the latter(P<0.05);3)Accuracy rate:the accuracy rate of CEUS in diagnosing both diseases was significantly higher than CDU’s one(96.25% vs81.25%,P<0.05). Conclusion In differentially diagnosing both diseases both inspections have certain value,only CEUS is better than CDU.
