摘  要:目的探讨血清神经丝蛋白轻链(NFL)水平与脑小血管病(CSVD)病人认知功能障碍的关系。方法2019年6月至2021年6月前瞻性收集CSVD共186例,用简易精神状态检查量表评估认识功能,ELISA检测血清NFL水平。结果186例中,存在认知功能障碍104例,认知功能正常82例。认知功能障碍发生率为55.91%。多因素lgistic回归分析显示,血清NFL水平(OR=3.128;95%CI 1.361~6.021,P=0.012)是CSVD病人认知功能障碍的独立危险因素。ROC曲线分析显示,血清NFL评估CS⁃VD病人认知功能障碍的曲线下面积为0.84(95%CI 0.712~0.921)。结论CSVD病人认知功能障碍发生率较高,血清NFL水平与CSVD病人认知功能障碍密切相关。Objective To explore the relationship between the serum level of NFL and the cognitive dysfunction of patients with cerebral small vessel disease(CSVD).Methods A Total of 186 patients with CSVD was recruited prospectively from June 2019 to June 2021.The cognitive function was assessed by MMSE.The serum level of NLF was detected using ELISA.Results Of 186 patients with CSVD,104 patients suffered from cognitive dysfunction.The rtae of cognitive dysfunction was 55.91%.Multivariate logistic analysis showed that the serum level of NFL was an independent risk factor for cognitive dysfunction of patients with CSVD(OR=3.128;95%CI 1.361~6.021;P<0.001).ROC curve analysis showed that the AUC of serum NFL for the diagnosis of cognitive dysfunction of CSVD patients was 0.84(95%CI:0.712 to 0.921).Conclusions Cognitive dysfunction is common in CSVD patients.Serum NFL levels are closely related to the cognitive dysfunction of CSVD patients.
