50例晚期恶性肿瘤患者免疫治疗副反应的临床观察

摘　　要：目的免疫检查点抑制剂(immune checkpointinhibitors,ICIs)彻底改变了癌症的治疗模式,在临床中得到广泛的应用,免疫治疗相关副反应(immune-related adverse events,irAEs)也逐渐引起人们的关注,本研究旨在观察免疫治疗在晚期恶性肿瘤中引起的毒性反应谱及治疗后转归。方法回顾性分析2020年1月至2022年1月在我科进行PD-1免疫单药或免疫联合(化疗或靶向)治疗的50例晚期恶性肿瘤患者的临床病理资料,总结治疗后出现的毒副反应类型、发生率及转归。结果50例免疫治疗患者中13例(26%)出现甲状腺功能减退,3例(6%)免疫性肺炎,1例(2%)免疫性肝炎,1例(2%)III°房室传导阻滞,1例(2%)糖尿病酮症酸中毒,1例(2%)动眼神经炎,7例(14%)出现反应性毛细血管增生症。结论免疫治疗副反应涉及各个系统,严重不良反应甚至危及生命,需提高警惕,尽早干预。Objective Immune checkpoint inhibitors(ICIs) have revolutionized the treatment of cancers and are widely used in clinical practice.The study was to investigate the toxicity spectrum induced by immunotherapy in the treatment of advanced tumors.Methods A retrospective analysis was performed on the types and incidence of adverse events after immunotherapy in 50 patients with advanced malignant tumor admitted to our department from January 2020 to January 2022.Results Of the 50 patients,13(26%) had hypothyroidism,3(6%) had pneumonia,1(2%) had hepatitis,1(2%) had Ⅲ° atrioventricular block,1(2%) had diabetic ketoacidosis,1(2%) had oculomotor neuritis,7(14%) had reactive capillary hyperplasia.Conclusion Immune-related adverse events involve various systems and serious adverse reactions are even life-threatening,which requires vigilance and early intervention.
