口腔种植修复牙列缺损的疗效观察及并发症危险因素分析

摘　　要：目的:探讨种植修复在牙列缺损患者中的修复效果,并分析其并发症发生的影响因素。方法:选取2016年6月-2020年6月笔者医院就诊的115例牙列缺损患者为研究对象。所有患者均行种植修复治疗,评价患者修复效果,记录并发症发生情况。采用单因素分析和多元Logistic回归分析并发症发生的危险因素。结果:115例患者中,成功64例,良好23例,一般17例,无效11例,总有效率为90.43%;修复体松动5例,修复体折断1例,固位丧失1例,邻接丧失4例,食物嵌塞8例,疼痛4例,感染2例,出血1例,并发症总发生率为22.61%。年龄、吸烟嗜好、糖尿病、骨量、缺牙颗数、种植位置、修复体材料均与并发症的发生有关(P<0.05)。多因素Logistic分析显示,吸烟嗜好、糖尿病、骨量不足、缺牙颗数≥3颗、修复体材料为全瓷冠均是并发症发生的危险因素(P<0.05)。结论:种植修复在牙列缺损患者中具有良好的修复效果,且吸烟嗜好、糖尿病、骨量不足、缺牙颗数≥3颗、修复体材料为全瓷冠均是并发症发生的危险因素。Objective To investigate the effect of implant restoration in patients with dentition defect,and analyze the influencing factors of complications.Methods Total of 115 patients with dentition defect who visited our hospital from June 2016 to June 2020 were selected.All patients underwent implant restoration treatment.The restoration effect of patients was evaluated and the occurrence of complications were recorded.Univariate analysis and multiple logistic regression were used to analyze the risk factors of complications.Results Among 115 patients,64 cases were successful,23 cases were good,17 cases were general,11 cases were ineffective,and the total effective rate was 90.43%;the restoration was loose in 5 cases,the restoration was broken in 1 case,retention was lost in 1 case,abutment was lost in 4 cases,food impaction in 8 cases,pain in 4 cases,infection in 2 cases,bleeding in 1 case,and the total occurrence of complications was 22.61%.Age,smoking habit,diabetes mellitus,bone mass,number of missing teeth,implant location and restoration materials were all related to the occurrence of complications (P<0.05).Multivariate logistic analysis showed that smoking habit,diabetes mellitus,insufficient bone mass,number of missing teeth≥3,and restoration material made of all-ceramic crowns were all risk factors for complications (P<0.05).Conclusion Implant restoration has a good restoration effect in patients with dentition defect,and smoking habit,diabetes mellitus,insufficient bone mass,number of missing teeth≥3,and restoration material made of all-ceramic crowns were all risk factors for complications.
