摘 要:目的 分析甲基乙二醛(MG)、25-羟维生素D3[25(OH)D3]、CXC趋化因子配体16(CXCL16)对高血压脑梗死合并认知功能障碍的评估价值。方法 收集2018年10月至2020年12月河北大学附属医院收治的199例高血压脑梗死患者的临床资料,根据是否合并认知功能障碍分为合并认知功能障碍者(合并组),未合并认知功能障碍者(未合并组),比较两组MG、25(OH)D3、CXCL16水平变化情况及蒙特利尔认知评分(MoCA)。分析MG、25(OH)D3、CXCL16与认知功能的相关性,并绘制受试者工作特征(ROC)曲线分析上述因子对高血压脑梗死合并认知功能障碍的预测价值。结果 199例患者中,合并认知功能障碍者102例(合并组),未合并认知功能障碍者97例(未合并组)。合并组MG、CXCL16水平明显高于未合并组,25(OH)D3水平、MoCA评分明显低于未合并组,差异有统计学意义(P<0.05)。Logistic回归分析发现,MG、25(OH)D3、CXCL16是高血压脑梗死合并认知功能障碍的独立危险因素。相关性分析结果显示,MG、CXCL16与MoCA评分呈负相关(P<0.05),25(OH)D3与MoCA评分呈正相关关系(P<0.05)。ROC曲线结果显示,MG+25(OH)D3+CXCL16三者联合检测对高血压合并AMI预测敏感度和特异度分别为0.903、0.865,AUC=0.890,明显高于各项指标单独预测(P<0.05)。结论 MG、25(OH)D3、CXCL16与高血压脑梗死认知功能密切相关,在临床中监测三者水平变化有助于高血压脑梗死患者认知功能障碍的临床诊断。Objective To analyze the evaluation value of methylglyoxal(MG),25-hydroxyvitamin D3[25(OH)D3],CXC chemokine ligand 16(CXCL16)in patients with hypertensive cerebral infarction combined with cognitive dysfunction. Methods The clinical data of 199 patients with hypertensive cerebral infarction admitted to the Affiliated Hospital of Hebei University from October 2018 to December 2020 were collected. According to whether they had the combined cognitive dysfunction,they were divided into patients with cognitive dysfunction(combined group)and those without cognitive dysfunction(uncombined group),and the changes in MG,25(OH)D3,CXCL16 levels and Montreal Cognitive Assessment(MoCA)score were compared between the two groups. The correlations between MG,25(OH)D3,CXCL16 and cognitive function were analyzed,and receiver operating characteristic(ROC)curves were drawn to analyze the predictive value of the above factors for hypertensive cerebral infarction complicated with cognitive dysfunction. Results Among the 199 patients,102 patients had cognitive impairment(combined group),and 97 patients had no cognitive impairment(uncombined group). The levels of MG and CXCL16 in the combined group were significantly higher than those in the uncombined group,and the 25(OH)D3 level and Mo CA score were significantly lower than those in the uncombined group,the differences were statistically significant(P<0.05). Logistic regression analysis showed that MG,25(OH)D3 and CXCL16 were independent risk factors for hypertensive cerebral infarction complicated with cognitive dysfunction. Correlation analysis results showed that MG,CXCL16 were negatively correlated with Mo CA score(P<0.05),and 25(OH)D3 was positively correlated with Mo CA score(P<0.05). The ROC curve showed that the sensitivity and specificity of the three combined(MG+25(OH)D3+CXCL16)detection for hypertension combined with AMI were 0.909,0.865,respectively;AUC=0.890,which was significantly higher than the prediction of each indicator alone(P<0.05).
