更昔洛韦眼凝胶与干扰素滴眼液治疗小儿单疱病毒性角膜炎的临床效果分析

摘　　要：目的探讨更昔洛韦眼凝胶与干扰素滴眼液治疗小儿单疱病毒性角膜炎的临床效果。方法72例小儿单疱病毒性角膜炎患儿,随机分为干扰素组和眼凝胶联合组,各36例。干扰素组患儿给予干扰素滴眼液治疗,眼凝胶联合组患儿给予干扰素滴眼液联合更昔洛韦眼凝胶治疗。比较两组临床疗效,症状及视力改善情况,炎症因子[干扰素-α(IFN-α)、白细胞介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)]水平,不良反应发生情况。结果眼凝胶联合组总有效率97.22%高于干扰素组的77.78%,差异具有统计学意义(P<0.05)。眼凝胶联合组眼部刺激征消退时间、角膜充血消退时间、溃疡愈合时间分别为(6.47±1.96)、(7.02±1.58)、(12.98±4.27)d,均短于干扰素组的(10.39±2.16)、(12.39±2.56)、(22.03±3.26)d,差异具有统计学意义(P<0.05)。治疗前,两组视力比较,差异无统计学意义(P>0.05);治疗后,眼凝胶联合组视力(0.91±0.18)高于干扰素组的(0.81±0.15),差异具有统计学意义(P<0.05)。治疗前,两组IFN-α、IL-8、TNF-α、CRP水平比较,差异无统计学意义(P>0.05);治疗后,眼凝胶联合组IFN-α(117.02±8.69)ng/L高于干扰素组的(89.69±7.59)ng/L,IL-8(50.03±5.96)ng/L、TNF-α(29.69±10.56)ng/L、CRP(3.69±0.86)mg/L低于干扰素组的(72.69±8.67)ng/L、(44.12±9.68)ng/L、(8.59±0.78)mg/L,差异具有统计学意义(P<0.05)。结论在干扰素滴眼液治疗基础上联合更昔洛韦眼凝胶局部涂抹,可有效提升小儿单疱病毒性角膜炎的治疗效果,促进眼部症状消退和溃疡愈合,提升炎症反应控制效果,临床应用安全可靠。Objective To discuss the clinical effect of ganciclovir eye gel and interferon eye drops in the treatment of children with herpes simplex keratitis.Methods A total of 72 children with herpes simplex keratitis were randomly divided into interferon group and eye gel combination group,with 36 cases in each group.Children in the interferon group were treated with interferon eye drops,and children in the eye gel combination group were treated with interferon eye drops combined with ganciclovir eye gel.The clinical efficacy,symptoms and visual acuity improvement,inflammatory factors[interferon-α(IFN-α),interleukin-8(IL-8),tumor necrosis factor-α(TNF-α),C-reactive protein(CRP)],and the occurrence of adverse reactions were compared between the two groups.Results The total effective rate 97.22% in the eye gel combination group was higher than 77.78%in the interferon group,and the difference was statistically significant(P<0.05).The eye irritation subsidence time,corneal congestion subsidence time,and ulcer healing time in the eye gel combination group were(6.47±1.96),(7.02±1.58),and(12.98±4.27)d,which were shorter than(10.39±2.16),(12.39±2.56),and(22.03±3.26)d in the interferon group,and the differences were statistically significant(P<0.05).Before treatment,there was no statistically significant difference in visual acuity between the two groups(P>0.05).After treatment,the visual acuity(0.91±0.18)of the eye gel combination group was higher than(0.81±0.15)of the interferon group,and the difference was statistically significant(P<0.05).Before treatment,there was no statistically significant difference in IFN-α,IL-8,TNF-α,CRP between the two groups(P>0.05).After treatment,the IFN-α(117.02±8.69)ng/L in eye gel combination group was higher than(89.69±7.59)ng/L in interferon group,and the IL-8(50.03±5.96)ng/L,TNF-α(29.69±10.56)ng/L and CRP(3.69±0.86)mg/L were lower than(72.69±8.67)ng/L,(44.12±9.68)ng/L and(8.59±0.78)mg/L in interferon group.All the differences were statistically significant(P<0.05).Conc
