摘 要:目的探究氧化锆桩核与金属桩核修复前牙残根的临床疗效,为前牙残根的修复提供参考。方法80例前牙残根患者,依据随机数字表法分为观察组与对照组,各40例。对照组患者予以铸造钴铬桩核+钴铬烤瓷全冠修复治疗,观察组患者予以氧化锆桩核+氧化锆全瓷冠修复治疗。对比两组治疗1年后桩核冠损情况、修复效果、满意度以及治疗前后牙周状况。结果治疗1年后,观察组修复体完整率100.00%、边缘适合率97.50%、颜色协调率97.50%均高于对照组的77.50%、80.00%、80.00%,差异有统计学意义(P<0.05)。观察组患者总满意度97.50%高于对照组的80.00%,桩核冠损发生率2.50%低于对照组的15.00%,差异有统计学意义(P<0.05)。治疗1年后,两组牙龈出血指数、牙周袋深度及附着丧失程度均较本组治疗前改善,差异具有统计学意义(P<0.05);两组牙龈出血指数、牙周袋深度及附着丧失程度比较,差异无统计学意义(P>0.05)。结论采用氧化锆桩核进行前牙残根牙体修复,可有效保障患者的牙体完整程度,颜色协调,桩核冠损伤少,修复整体效果理想,促进患者预后。Objective To investigate the clinical efficacy of zirconia post-and-core and metal post-andcore in restoration of residual roots of anterior teeth,so as to provide reference for restoration of residual roots of anterior teeth.Methods A total of 80 patients with residual roots of anterior teeth were divided into observation group and control group according to the random numerical table,with 40 cases in each group.Patients in the control group were treated with casting cobalt-chromium post-and-core+cobalt-chromium porcelain crowns,while the observation group were treated with zirconia post-and-core+zirconia all-ceramic crowns.The postand-core crown damage after 1 year of treatment,restoration effect,satisfaction,periodontal status before and after treatment were compared between the two groups.Results After 1 year of treatment,the restoration integrity rate 100.00%,edge fit rate 97.50%,and color coordination rate 97.50% in the observation group were higher than 77.50%,80.00%,and 80.00% in the control group,and the differences were statistically significant(P<0.05).The total satisfaction rate 97.50% of the observation group was higher than 80.00% of the control group,and the incidence of post-and-core crown damage 2.50% was lower than 15.00% of the control group.All the differences were statistically significant(P<0.05).After 1 year of treatment,the gingival bleeding index,periodontal pocket depth and periodontal attachment loss of the two groups were improved compared with those of this group before treatment,and the difference was statistically significant(P<0.05).There was no statistically significant difference in gingival bleeding index,periodontal pocket depth and periodontal attachment loss between the two groups(P>0.05).Conclusion Zirconia post-and-core for restoration of residual roots of anterior teeth can effectively guarantee the patient's dental integrity and color coordination,with less damage to the post-and-core crown and ideal overall restoration results,and promote the patient's prognosis.
