摘  要:强迫障碍(obsessive-compulsive disorder,OCD)临床上主要表现为强迫思维和强迫行为,具有发病年龄早、病程长及诊治率低等特征。该疾病涉及5-羟色胺、谷氨酸及多巴胺系统相关基因变异以及相关脑区结构和功能的改变。这些潜在致病基因如何影响脑结构和功能,进而导致强迫障碍表现出不同的临床症状仍未有定论。影像遗传学通过将遗传学和影像学结合,可关联分析遗传变异、脑神经和临床表现的关系,目前已被应用于强迫障碍的病因学机制研究。作为复杂的多基因疾病,结合多基因关联分析或表观遗传的影像遗传学可能成为探讨强迫障碍中环境-基因-脑-行为模型机制的新趋势和新角度。因此,本文对强迫障碍的影像遗传学相关研究进行综述,以期为强迫障碍的病因学机制研究及临床精准干预提供理论依据。Obsessive-compulsive disorder(OCD)mainly manifests as obsessions and compulsions with the characteristics of early age of onset,long illness duration,and low diagnosis and treatment rate.The development of OCD involves mutations in genes associated with the 5-hydroxytryptamine,glutamate,and dopamine systems,as well as changes in the brain structure and function.However,how the potentially pathogenic genes affect brain structure and function and lead to different clinical manifestations remains unknown.Imaging genetics has been used to study the etiological mechanisms of OCD by combining genetics and imaging to analyze the relationship between genetic variation,neurologic and clinical manifestations.As a complex polygenic disease,imaging genetics that combines polygenic association analysis with epigenetic inheritance may become a new trend and perspective to explore the mechanism of the environment-gene-brain-behavior model in OCD.Therefore,this paper reviewed the related imaging genetics studies of OCD,and provided a theoretical basis for the etiological mechanism and precise intervention for OCD.
