摘 要:目的探讨抑郁症患者血清肥胖抑制素、摄食调节因子-1(nesfatin-1)水平与认知功能的相关性。方法纳入60例抑郁症患者作为观察组,另选取60例健康正常人作为对照组。检测并比较两组血清肥胖抑制素、nesfatin-1水平,采用17项汉密尔顿抑郁量表(HAMD-17)评估观察组的抑郁状况,采用MATRICS共识认知成套测验(MCCB)评估其认知功能。采用Pearson相关性分析抑郁症患者血清肥胖抑制素、nesfatin-1水平与HAMD-17、MCCB评分之间的关系。应用Logistic回归分析影响抑郁患者认知功能损伤的危险因素。ROC曲线分析血清肥胖抑制素、nesfatin-1表达水平对抑郁症患者认知功能损伤的诊断价值。结果与对照组比较,观察组血清肥胖抑制素、nesfatin-1水平均显著升高,差异有统计学意义(P<0.05)。与认知功能正常组比较,认知功能损伤组的HAMD-17评分、血清肥胖抑制素及nesfatin-1水平均显著升高,MCCB评分显著降低,差异有统计学意义(P<0.05)。多因素Logsitic回归分析表明,血清肥胖抑制素、nesfatin-1水平升高是抑郁症患者发生认知功能损伤的危险因素(P<0.05)。ROC曲线显示,血清肥胖抑制素水平诊断抑郁症患者认知功能损伤的曲线下面积(AUC)为0.755(95%CI:0.628~0.881),nesfatin-1表达水平诊断抑郁症患者认知功能损伤的AUC为0.785(95%CI:0.667~0.904)。结论肥胖抑制素、nesfatin-1可能是诊断抑郁症患者发生认知功能损伤的生物标志物。Objective To investigate the levels of serum obestatin and appetite regulatory factor-1(nesfatin-1)in patients with depression,and their relationship with cognitive function.Methods A total of 60 patients with depression were included in the study as the observation group,and 60 healthy normal people as the control group.The levels of serum obestatin and nesfatin-1 were detected and compared between the two groups.17 Hamilton Depression Scale(HAMD-17)was used to evaluate the depression status of depression group,and MATRICS Consensus Cognitive Test(MCCB)was used to evaluate cognitive function.Pearson correlation analysis of the relationship between serum obestatin,nesfatin-1 levels and HAMD-17,MCCB scores in patients with depression.Logistic regression analysis was used to analyze risk factors of cognitive impairment in depressed patients.ROC curve analysis was used to analyze the value of serum obestatin and nesfatin-1 expression levels in the diagnosis of cognitive impairment in patients with depression.Results Compared with the control group,the levels of serum obestatin and nesfatin-1 in the obeservation group were increased,with statistical significance(P<0.05).Compared with the normal cognitive function group,the scores of HAMD-17,serum obestatin and nesfatin-1 in the cognitive function impairment group were significantly increased,while the scores of MCCB were significantly decreased,with statistical significance(P<0.05).Multivariate Logistic regression analysis showed that high serum levels of obestatin and nesfatin-1 were risk factors for cognitive impairment in patients with depression(P<0.05).ROC curve showed that the area under the curve(AUC)of serum obestatin level in diagnosing cognitive impairment of depression patients was 0.755(95%CI:0.628~0.881),and the AUC of nesfatin-1 expression in diagnosing cognitive impairment of depression patients was 0.785(95%CI:0.667~0.904).Conclusion Obestatin and nesfatin-1 may be a biomarker for the diagnosis of cognitive impairment in patients with depression.
