浅表韧带样纤维瘤的超声二维及超声造影表现

摘　　要：目的分析浅表韧带样纤维瘤的超声表现,提高超声对浅表韧带样纤维瘤的诊断准确性。方法回顾性收集经手术或穿刺后病理证实的浅表韧带样纤维瘤患者15例,分析其超声二维及超声造影(CEUS)表现。结果15例浅表韧带样纤维瘤均为单发低回声肿块,肿块均位于浅表肌层内,多位于腹壁肌层,肿块边界多为欠清或不清,形态多为欠规则或不规则,彩色多普勒超声(CDFI)多为中量及以上血流信号,超声造影多表现为高增强,增强模式多呈快进慢出,但部分病例呈快进快出。结论浅表韧带样纤维瘤以中青年女性较为多见,且多位于腹壁肌层,超声表现具有一定特征性,结合临床病史可提高诊断准确性。Objective To analyze the ultrasonographic features of superficial desmoid-type fibromatosis(DF),and to improve the diagnostic accuracy of ultrasound for superficial DF.Methods Fifteen patients with superficial DF confirmed by surgery or puncture were retrospectively collected,and their two-dimensional ultrasound and contrast-enhanced ultrasound(CEUS)findings were analyzed.Results The 15 cases of superficial DF were all single hypoechoic masses.The masses were located in the superficial muscle layer,most of which were located in the abdominal wall muscle layer,the boundary of the mass was mostly unclear or unclear,and the shape was mostly irregular or irregular.Color Doppler ultrasound(CDFI)was mostly moderate and above blood flow signals.CEUS mostly showed high enhancement,and the enhancement mode was mostly fast in and slow washed out,but some cases showed fast in and fast washed out.Conclusion Superficial DF is more common in young and middle-aged women,and most of them are located in the muscular layer of abdominal wall.Ultrasound findings of superficial DF has certain characteristics,and it combined with clinical history can improve the accuracy of diagnosis.
