耳穴压豆对脑卒中吞咽功能障碍患者吞咽功能康复及生活质量的影响

摘　　要：目的:观察耳穴压豆对脑卒中吞咽功能障碍患者吞咽功能康复及生活质量的影响。方法:将90例脑卒中吞咽功能障碍患者按照随机数字表法分为观察组和对照组各45例,2组均予康复医学科规范化治疗与吞咽功能训练,观察组联合耳穴压豆治疗。2组均治疗3个月。治疗前后评定2组患者的标准吞咽功能评价量表(SSA)评分、洼田饮水试验评分、神经功能缺损评分[美国国立卫生院卒中量表(NIHSS)]、Barthel指数(BI)评分及吞咽障碍特异性生活质量量表(SWAL-QOL)评分,比较2组的临床疗效,观察不良反应发生情况。结果:治疗后,观察组临床疗效总有效率为91.1%,高于对照组的73.3%(P<0.05)。2组SSA评分、洼田饮水试验评分、NIHSS评分均较治疗前下降(P<0.05),BI评分、SWAL-QOL评分均较治疗前上升(P<0.05);观察组SSA评分、洼田饮水试验评分、NIHSS评分均低于对照组(P<0.05),BI评分、SWAL-QOL评分均高于对照组(P<0.05)。2组患者在治疗期间均未出现与药物相关的严重不良反应。结论:耳穴压豆对脑卒中吞咽功能障碍患者辅助治疗效果明显,有助于改善患者的吞咽功能及日常活动能力,提高生活质量,促进疾病康复。Objective:To observe the effect of auricular point pressing with beans on swallowing rehabilitation and quality of life of patients with post-stroke dysphagia.Methods:A total of 90 patients with post-stroke dysphagia were divided into the observation group and the control group according to random number table,with 45 cases in each group.Both groups were given standardized treatment in Rehabilitation Medicine Department and swallowing function training,and the observation group was additionally treated with auricular point pressing with beans.Both groups were treated for three months.The scores of Standardized Swallowing Assessment(SSA),drinking water test,National Institutes of Health Stroke Scale(NIHSS),Barthel Index(BI)and Swallowing-Quality of Life(SWAL-QOL)were evaluated before and after treatment.The clinical effects in the two groups were compared and the occurrence of adverse reactions was observed.Results:After treatment,the total effective rate was 91.1%in the observation group,higher than that of 73.3%in the control group(P<0.05).The scores of SSA,drinking water test and NIHSS in both groups were decreased when compared with those before treatment(P<0.05),and the scores of BI and SWAL-QOL were increased(P<0.05).The scores of SSA,SSA,drinking water test and NIHSS in the observation group were lower than those in the control group(P<0.05),and the scores of BI and SWAL-QOL were higher(P<0.05).There was no serious adverse reaction related to medicine in both groups during treatment.Conclusion:Auricular point pressing with beans has a significant therapeutic effect on patients with post-stroke dysphagia,which helps improve the swallowing function and daily activity ability of patients,enhance the quality of life and promote disease rehabilitation.
