摘 要:目的探讨血清肿瘤特异性生长因子(TSGF)、巨噬细胞移动抑制因子(MIF)水平对骨肉瘤(OS)患者免疫靶向治疗效果的预测价值。方法选取72例OS患者,均采用α-干扰素联合贝伐珠单抗治疗12周,根据疗效将患者分为有效组和无效组。检测所有患者治疗前后的血清TSGF、MIF水平,比较无效组和有效组OS患者的基线特征,采用Logistic回归模型分析OS患者免疫靶向治疗效果的影响因素,分析血清TSGF、MIF单独及联合检测对OS患者免疫靶向治疗疗效的预测价值。结果治疗12周结束时,OS患者的血清TSGF、MIF水平均明显低于治疗前(P﹤0.01)。72例患者中,治疗有效53例(73.61%),治疗无效19例(26.39%)。无效组OS患者治疗前碱性磷酸酶(ALP)、TSGF、MIF水平均明显高于有效组患者,差异均有统计学意义(P﹤0.01)。Logistic回归分析结果显示,治疗前血清ALP、TSGF、MIF水平均是OS患者免疫靶向治疗效果的影响因素(P﹤0.01)。受试者工作特征(ROC)曲线分析结果显示,血清TSGF、MIF单独及联合检测预测OS患者免疫靶向治疗效果的曲线下面积(AUC)分别为0.825、0.805、0.914,联合检测的预测价值高于单独检测。结论部分OS患者经免疫靶向治疗无效,可能与患者治疗前血清TSGF、MIF水平较高有关,考虑未来可将TSGF、MIF作为辅助评估OS患者免疫靶向治疗效果的重要指标。Objective To investigate the predictive value of serum tumor specific growth factor(TSGF)and macro-phage migration inhibitory factor(MIF)levels on the effect of immune-targeted therapy in patients with osteosarcoma(OS).Method A total of 72 patients with OS were selected,and all patients were treated withα-interferon combined with bevacizumab for 12 weeks.The patients were divided into effective group and ineffective group according to the curative effect.Serum TSGF and MIF levels were detected in all patients before and after the treatment.The baseline data of OS patients in the ineffective group and effective group were also compared.Logistic regression model was built to analyze the risk factors influencing the effect of immune-targeted therapy in OS patients.The predictive value of serum TSGF and MIF detection alone and in combination on the effect of immune-targeted therapy in OS patients were also analyzed.Result At the end of 12 weeks of treatment,the serum TSGF and MIF levels of OS patients were significantly lower than those before treatment(P<0.01).Among the 72 patients,53(73.61%)achieved response and 19(26.39%)did not.The levels of alkaline phosphatase(ALP),TSGF,and MIF in the OS patients in the ineffective group before the treatment were significantly higher than those in the effective group,and the differences were statistically significant(P<0.01).Logistic regression analysis showed that serum ALP,TSGF,and MIF levels before the treatment were risk factors for the effect of immune-targeted therapy in OS patients(P<0.01).The results of the receiver operating characteristic(ROC)curve analysis showed that the area under the curve(AUC)of TSGF and MIF alone and in combination in predicting the effect of immune-targeted therapy in OS patients were 0.825,0.805,and 0.914,respectively,and the predictive value of combined detection was higher than that of the single detection.Conclusion A part of OS patients are insensitive after immune-targeted therapy,which may be related to the high levels of serum TSGF and
