复方八珍汤辅助治疗口腔种植体周围炎临床研究

摘　　要：目的:观察复方八珍汤辅助治疗口腔种植体周围炎的临床疗效及对龈沟液炎症因子水平的影响。方法:将60例口腔种植体周围炎患者随机分为观察组(31例)与对照组(29例)。对照组接受常规西医治疗,观察组在对照组基础上采取复方八珍汤治疗。比较2组治疗前后中医证候积分、龈沟液炎症因子[白细胞介素-8(IL-8)、白细胞介素-6(IL-6)、白细胞介素-1β(IL-1β)]、牙周学相关指标[龈沟出血指数(SBI)、菌斑指数(PLI)、牙周袋深度(PD)],并评价2组临床疗效及不良反应发生情况。结果:观察组总有效率93.55%,高于对照组72.41%(P<0.05)。治疗后,2组中医证候积分、SBI、PLI、PD及龈沟液IL-8、IL-6、IL-1β水平均较治疗前降低(P<0.05),且观察组治疗后上述指标均低于对照组(P<0.05)。2组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:复方八珍汤辅助治疗口腔种植体周围炎疗效确切,可明显降低患者龈沟液炎症因子,抑制牙菌斑生长,改善出血症状,安全性较好。Objective:To observe the clinical effect of compound Bazhen tang on oral peri-implantitis and its effect on levels of inflammatory factors in gingival crevicular fluid.Methods:A total of 60 patients with oral peri-implantitis were randomly divided into the observation group and the control group,with 31 and 29 cases in each group respectively.The control group was treated with routine western medicine,and the observation group was additionally treated with compound Bazhen tang based on the treatment of the control group.Before and after treatment,the Chinese medicine syndrome scores,inflammatory factors in gingival crevicular fluid,including interleukin-8(IL-8),interleukin-6(IL-6)and interleukin-1β(IL-1β),and related indexes of periodontology,including sulcus bleeding index(SBI),plaque index(PLI)and pocket depth(PD),were compared between the two groups,and the clinical effects and adverse reactions in the two groups were evaluated.Results:The total effective rate was 93.55%in the observation group,higher than that of 72.41%in the control group(P<0.05).After treatment,the Chinese medicine syndrome scores and levels of SBI,PLI,PD,and IL-8,IL-6,and IL-1βin gingival crevicular fluid in the two groups were decreased when compared with those before treatment(P<0.05),and the above indexes in the observation group were lower than those in the control group(P<0.05).There was no significant difference being found in the comparison of incidence of adverse reactions between the two groups(P>0.05).Conclusion:The therapy of compound Bazhen tang has a positive curative effect on oral peri-implantitis,which can significantly reduce the inflammatory factors in gingival crevicular fluid,inhibit the growth of dental plaque and improve the symptom of bleeding,with good safety.
