miRNA-145和miRNA-200c在高级别宫颈上皮内瘤变与宫颈癌组织中的表达及其临床意义

摘　　要：目的比较高级别宫颈上皮内瘤变(CIN)与宫颈癌组织中微小RNA-145(miR-145)和微小RNA-200c(miR-200c)的表达水平。方法抽取山西省肿瘤医院2019年2月至2020年3月收治的80例行锥切手术治疗的CIN患者作为CIN组,根据宫颈病变程度将其分为Ⅰ级组(24例)、Ⅱ级组(34例)、Ⅲ级组(22例)。另选择同期40例行根治手术的宫颈癌患者作为宫颈癌组。收集锥切术后留取CIN组织、根治手术存档宫颈癌组织及癌旁正常组织(距离癌灶>3 cm)进行miR-145和miR-200c水平检测。比较miR-145和miR-200c在CIN组织、宫颈癌组织和癌旁正常组织中的表达水平,不同级别的CIN组织中miR-145和miR-200c的表达水平,不同临床病理参数的宫颈癌患者病变组织中miR-145和miR-200c的表达水平。结果宫颈癌组织中miR-145和miR-200c的表达水平为0.40±0.06、0.35±0.09,CIN组织中miR-145和miR-200c的表达水平为0.78±0.20、0.35±0.09,低于癌旁组织的1.00±0.11、1.00±0.15(P<0.05),且宫颈癌组织中miR-145和miR-200c的表达水平低于CIN组织(P<0.05)。Ⅲ级组CIN组织中miR-145、miR-200c的表达水平低于Ⅰ级组和Ⅱ级组(P<0.05)。合并淋巴结转移、低分化、肿瘤长径>4 cm的宫颈癌患者宫颈组织中miR-145、miR-200c的表达水平低于未合并淋巴结转移、高分化、长径≤4 cm的宫颈癌患者(P<0.05)。结论miR-145和miR-200c在CIN、宫颈癌组织中表达下调,其表达水平随CIN级别升高而下降,二者可能参与宫颈癌的发生、发展过程。Objective To compare the expression levels of microRNA-145(miR-145)and microRNA-200c(miR-200c)in high-grade cervical intraepithelial neoplasia(CIN)and cervical cancer tissues.Methods Eighty CIN patients who underwent conization surgery in Shanxi Cancer Hospital from February 2019 to March 2020 were selected as CIN group.According to the degrees of cervical lesions,they were divided into gradeⅠgroup(24 cases),gradeⅡgroup(34 cases)and gradeⅢgroup(22 cases).Another 40 patients with cervical cancer who underwent radical surgery during the same period were selected as cervical cancer group.The CIN tissues were collected after conization,and the cervical cancer tissues and adjacent normal tissues(>3 cm away from tumor)were collected after radical surgery for the detection of miR-145 and miR-200c levels.The expression levels of miR-145 and miR-200c in CIN tissues,cervical cancer tissues,and adjacent normal tissues were compared;the expression levels of miR-145 and miR-200c in different grades of CIN tissues were compared;levels of miR-145 and miR-200c in CIN tissues were compared among cervical cancer patients with different clinicopathological parameters.Results The expression levels of miR-145 and miR-200c in cervical cancer tissues were 0.40±0.06 and 0.35±0.09,respectively,and those in CIN tissues were 0.78±0.20 and 0.35±0.09,respectively,which wereⅠower than the 1.00±0.11 and 1.00±0.15 in adjacent tissues(P<0.05);and the expression levels of miR-145 and miR-200c in cervical cancer tissues were lower than those in CIN tissues(P<0.05).The expression levels of miR-145 and miR-200c in CIN tissue in gradeⅢgroup were lower than those in gradeⅠgroup and gradeⅡgroup(P<0.05).The expression levels of miR-145 and miR-200c in cervical tissue of patients with lymph node metastasis,poorly differentiated,and tumor major diameter>4 cm were lower than those in patients without lymph node metastasis,but with high differentiation and tumor major diameter≤4 cm(P<0.05).Conclusions The expressions of miR-145 and miR
