摘  要:目的分析术中神经电生理监测技术在听神经瘤切除术中的应用价值。方法抽取驻马店市中心医院2020年8月至2021年6月收治的听神经瘤患者80例,按随机数字表法分为观察组与对照组,每组40例。对照组行常规显微外科听神经瘤切除术,观察组行神经电生理监测技术下听神经瘤切除术。比较两组肿瘤切除情况、面神经解剖和功能保留情况、术后面神经功能分级、术后听力功能和面瘫症状改善情况、并发症发生率。结果观察组肿瘤完全切除率为92.50%(37/40),高于对照组的75.00%(30/40),P<0.05;肿瘤部分切除率为7.50%(3/40),低于对照组的25.00%(10/40),P<0.05。观察组面神经解剖保留率为92.50%(37/40),面神经功能保留率为97.50%(39/40),高于对照组72.50%(29/40)、75.00%(30/40),P<0.05。观察组术后面神经功能分级、听力功能和面瘫症状改善情况优于对照组(P<0.05)。观察组并发症发生率为2.50%(1/40),低于对照组的20.00%(8/40),P<0.05。结论听神经瘤切除术中应用神经电生理监测技术,可提高肿瘤完全切除率和面神经解剖、功能保留率,利于改善术后面神经功能、听力功能和面瘫症状,并能有效降低并发症发生率,提高手术安全性。Objective To analyze the application value of neurophysiological monitoring technology in resection of acoustic neuroma.Methods A total of 80 patients with acoustic neuroma treated in Zhumadian Central Hospital from August 2020 to June 2021 were selected and divided into observation group and control group according to random number table method,with 40 cases in each group.The control group received conventional microsurgical acoustic neuroma resection,and the observation group received acoustic neuroma resection under neuroelectrophysiological monitoring technology.The tumor resection,facial nerve anatomy and function preservation,postoperative facial nerve function classification,postoperative hearing function,improvement of facial paralysis symptoms,and incidence of complications were compared between the two groups.Results The complete resection rate of tumor in the observation group was 92.50%(37/40),which was higher than the 75.00%(34/40)in the control group(P<0.05),and the partial resection rate of tumor was 7.50%(3/40),which was lower than the 25.00%(10/40)in the control group(P<0.05).The facial nerve anatomical and functional retention rates of the observation group were 92.50%(37/40)and 97.50%(39/40),respectively,which were higher than the 72.50%(29/40)and 75.00%(30/40)of the control group(P<0.05).The improvement of facial nerve function,hearing function and facial paralysis symptoms in the observation group were better than those in the control group(P<0.05).The incidence of complications in the observation group was 2.50%(1/40),which was lower than the 20.00%(8/40)in the control group(P<0.05).Conclusions The application of nerve electrophysiological monitoring technology in the resection of acoustic neuroma can improve the complete resection rate of tumor and the preservation rate of facial nerve anatomy and function,improve the postoperative facial nerve function,hearing function and facial paralysis symptoms,effectively reduce the incidence of complications,and improve the safety of operation.
