间歇性外斜视儿童近视矫正与融合功能的观察

摘　　要：目的比较间歇性外斜视儿童近视屈光矫正前后的融合性集合及融合控制力。方法42例能配合检查的近视性IXT儿童,用标准对数视力表查视力,诊室控制力评分评估远近距离融合控制力,三棱镜交替遮盖法测远近斜视度,底朝外(BO)的棱镜测量法测量远近正相对集合(PRC),经验光屈光矫正1月后复查以上检查,比较屈光矫正前后各测量值的差别。结果屈光矫正后的远控制力评分高于矫正前评分,远斜视度低于矫正前,远PRC高于矫正前,差异均有统计学意义(Z_(远控)=-5.642,P_(远控)<0.001,Z_(远斜)=-5.691,P_(远斜)<0.001,Z_(远PRC)=-5.715,P_(远PRC)<0.001);近控制力评分、近斜视度、PRC的差异无统计学意义(Z_(近控)=-1.000,P_(近控)=0.317,Z_(近斜)=-1.414,P_(近斜)=0.157,Z_(近PRC)=-1.706,P_(近PRC)=0.088)。结论近视性IXT儿童屈光矫正可能改善远距融合控制力及融合性集合。Objective To investigate the fusional convergence and fusional control of the myopic intermittent exotropia children.Methods By retrospective study,42 myopic intermittent exotropia children were involved.All of the children were no previous myoporthosis.Visual acuity was measured with standard logarithmic visual acuity chart.Fusional control was evaluated by the control force score in the clinic.Strabismus was measured by the prism.Positive relative convergenc(PRC)was measured by base-out prism.After one month of refractive correction,the above examinations were reviewed to compare the differences of measured values before and after myoporthosis.Results After myoporthosis,the far fusional control score was higher than that before myoporthosis,the far-strabismus was lower than that before myoporthosis,and the far PRC was higher than that before myoporthosis,with statistical significance(P<0.001);There were no significant differences in the score of near fusional control,near strabismus and PRC(P>0.05).Conclusion Myoporthosis in children with myopic IXT may improve fusional control and convergence.
