摘 要:目的分析乳腺导管内乳头状瘤疾病特点,研究临床诊断中应用超声造影的价值和意义,为疾病诊治工作提供参考。方法本次研究对象以我院收治于2018年3月至2019年2月的乳腺手术且术前行超声造影检测的患者88例为主,对其临床资料予以回顾性分析,发现有100个病灶,其中10例病理结果为导管内乳头状瘤,对上述患者实施超声造影,以不均匀以及均匀增强,区域及周边增强,还有无增强为主要诊断结果,对造影特征进行分析,并对比病理结果。结果5例患者经常规超声诊断出导管内乳头状瘤,以1.5~3.6 cm为病灶范围;另外有4例患者出现均匀性增强于超声造影诊断中,且病灶大小为0.4~1.0 cm。最终结果显示,导管内乳头状瘤患者中出现区域增强及均匀增强的分别为4例及5例,剩下1例患者无增强,概率分别为40.00%、50.00%及10.00%,病灶大小分别为(2.02±1.60)cm、(1.40±1.20)cm、0.2 cm。对于超声造影诊断,导管内乳头状瘤以病灶生长为树枝样,轴心为纤维血管,导管内乳头状突出物扩张为增强区主要病理学特点;而纤维组织及导管扩张则为非增强区特点。结论将超声造影应用于乳腺导管内乳头状瘤患者的疾病诊断中,可发现区域增强以及均匀增强,且组织病理学特征关联于超声造影表现,能够使疾病检出率提高,助于疾病早期治疗。Objective To analyze the characteristics of breast intraductal papilloma,to study the value and significance of the application of contrast-enhanced ultrasound in clinical diagnosis,and to provide reference for the diagnosis and treatment of the disease.Methods The subjects of this study were mainly 88 patients who were admitted to our hospital for breast surgery from March 2018 to February 2019 and underwent preoperative contrast-enhanced ultrasound examination.The clinical data was retrospectively analyzed,and it was found that there were 100patients.Among them,10 cases of pathological results were intraductal papilloma.The above-mentioned patients underwent contrast-enhanced ultrasound,with uneven and uniform enhancement,regional and peripheral enhancement,and whether enhancement was the main diagnostic result.The contrast characteristics were analyzed and the pathology was compared result.Results Five patients were diagnosed with intraductal papilloma by conventional ultrasound,with a lesion range of 1.5-3.6 cm;another 4 patients showed uniformity enhancement in the diagnosis of contrast-enhanced ultrasound,and had a lesion size of about 0.4-1.0 cm.The final results showed that 4 cases and 5 cases of intraductal papilloma had regional enhancement and uniform enhancement,respectively,and the remaining 1 patient had no enhancement.The probability was 40.00%,50.00%and 10.00%,respectively.The size of the lesion was respectively(2.02±1.60)cm,(1.40±1.20)cm,0.2 cm.For the diagnosis of contrastenhanced ultrasound,intraductal papilloma grows as dendritic lesions,and the axis is fibrous blood vessels.The expansion of intraductal papillary protrusions is the main pathological feature of the enhancement zone;while the fibrous tissue and duct expansion are the characteristics of the nonenhancement zone.Conclusion The application of contrast-enhanced ultrasound in the diagnosis of diseases of patients with breast intraductal papilloma can find regional enhancement and uniform enhancement,and histopathological characteris
