针刺治疗中重度干眼疗效的临床研究

摘　　要：目的探讨针刺单穴内睛明治疗中重度干眼的疗效。方法将60例干眼患者随机分为针刺组和药物组。共治疗8周,随访32周。观察指标为Schirmer-I试验(SIT)值、泪膜破裂时间(TBUT)、角膜荧光素染色(CFS)、眼表疾病指数(OSDI)和针刺可接受性评价。结果治疗8周后,SIT值针刺组明显高于药物组(P<0.05)。OSDI评分在8周和随访时组间有显著差异。泪膜破裂时间和角膜荧光素染色的变化在8周时没有显著差异,其中5例出现针刺相关不良事件。结论针刺单穴内睛明能显著促进泪液分泌,改善多种眼部不适症状。针灸对中重度干眼的疗效优于人工泪液。Objective To investigate the curative effect of acupuncture at a single point Neijingming in the treatment of moderate to severe dry eye disease(DED).Methods A total of sixty DED patients were randomly divided into an acupuncture group and an artificial tears(AT)group.Either acupuncture or AT was performed for an 8-week period,and a 32-week follow-up was performed.The outcome measures included the Schirmer-I test(SIT)value,the tear-film break-up time(TBUT),corneal fluorescein staining(CFS),the ocular surface disease index(OSDI),and the acupuncture acceptability.Adverse events were also monitored and documented.Results After 8 weeks of treatment,there was a significant difference in the SIT values between the acupuncture group and the artificial tears group(P<0.05).There was a significant difference in the OSDI scores between the two groups at the 8th week and the follow-up,while the changes in the tear film break-up time and corneal fluorescein staining were not significantly different at the 8th week,and five cases experienced acupuncture-related adverse events.Conclusion Acupuncture at a single point Neijingming can significantly promote tear secretion and improve multiple eye discomfort symptoms,which is more effective than artificial tears for moderate to severe dry eye disease.
