摘　　要：目的:系统评价元认知训练对精神分裂症病人认知自知力的疗效。方法:检索the Cochrane Library、PubMed、EMbase、中国知网(CNKI)、万方数据库(WanFang Database)、维普数据库(VIP)、中国生物医学文献数据库(CBM)数据库中公开发表的有关元认知训练与精神分裂症病人认知自知力相关的研究文献,检索时间为各数据库建库至2021年4月1日,由2名研究者独立筛选文献、提取资料并评价纳入研究的偏倚风险后,应用RevMan 5.4软件进行Meta分析。结果:共纳入8篇文献,全部为随机对照研究,样本总量为614例,元认知训练可显著提高精神分裂症病人的认知自知力[MD=4.22,95%CI(2.54,5.90),P<0.00001],增强自我反思能力[MD=2.40,95%CI(1.39,3.40),P<0.00001],降低自我确定[MD=-0.90,95%CI(-1.32,-0.48),P<0.0001]。结论:现有证据表明,元认知训练可以显著提高精神分裂症病人的认知自知力综合指数,提高病人自我反思能力及降低自我确定。但受纳入研究数量和质量的限制,上述结论尚待更多高质量研究予以验证。Objective:To systematically evaluate the effect of metacognitive training on cognitive insight in patients with schizophrenia.Methods:Related studies on metacognitive training and cognitive insight of schizophrenic patients were searched from the Cochrane Library,PubMed,EMbase,CNKI,WanFang Database,VIP and CBM.The retrieval time was from inception to April 1,2021.After Two researchers independently screened the literature,extracted data and evaluated the risk of bias included in the study,RevMan 5.4 software was used for Meta-analysis.Results:A total of 8 articles were included,all of which were randomized controlled studies,with a total sample of 614 cases.Metacognitive training could significantly improve cognitive insight[MD=4.22,95%CI(2.54,5.90),P<0.00001],enhance self-reflection ability[MD=2.40,95%CI(1.39,3.40),P<0.00001],and reduce self-determination[MD=-0.90,95%CI(-1.32,-0.48),P<0.0001].Conclusions:Current evidence shows that metacognitive training can significantly improve the comprehensive index of cognitive insight,improve the ability of self reflection and reduce self-determination in patients with schizophrenia.However,due to the limitations of the number and quality of included studies,the above conclusions need to be verified by more high-quality studies.
