玻璃纤维桩树脂核用于修复上颌前牙区缺损临床效果观察

摘　　要：目的:研究玻璃纤维桩树脂核用于修复上颌前牙区缺损的临床效果.方法:选择本院2018年5月~2021年3月诊治的87例上颌前牙区缺损患者,均接受修复治疗,按照随机数字表法分为研究组(44例)和对照组(43例),前者给予玻璃纤维桩树脂核修复,后者给予铸造金属桩核修复.比较两组患者修复成功率、美容效果、骨吸收量、吞咽功能、咀嚼功能,并统计修复后不良事件发生率.结果:术后6个月两组患者修复成功率、垂直和水平骨吸收量比较无差异,但术后12个月研究组患者修复成功率高于对照组,垂直、水平骨吸收量均低于对照组;两组患者唇侧牙龈高度评分比较无差异,但研究组根部突度与牙龈色质、远/近中龈乳头等评分显著高于对照组;术前两组患者吞咽、咀嚼功能比较无差异,术后研究组吞咽功能、咀嚼功能评级均高于对照组;两组患者术后根折/冠折、全冠脱落和根尖周病变发生率比较无差异,但研究组患者牙龈炎发生率低于对照组.结论:玻璃纤维桩树脂核应用于上颌前牙区修复中具有较好的远期效果和安全性,同时可提高患者美容效果、吞咽和咀嚼功能,减少骨吸收量.Objective This research aims to study the clinical effect of glass fiber post resin core in repairing maxillary an-terior teeth defect.Methods 87 patients with maxillary anterior tooth defects were enrolled in this research between May 2018 and March 2021.Using random number table method,the patients were divided into study group(44 cases)and control group(43 cases).The study group received restoration with glass fiber post and resin core,while the control group received restoration with cast metal post and core.The success rate of restoration,cosmetic effect,bone resorption,swallowing function,and masti-catory function were compared between the two groups.The incidence of adverse events after restoration was counted.Results There was no difference in the success rate of restoration,vertical or horizontal bone resorption between the two groups at 6 months after operation,12 months after surgery,the success rate of repair in the study group was significantly higher than that in the control group,and the vertical and horizontal bone absorption was lower than that in the control group.There was no differ-ence in labial gingival height score between the two groups,but the scores of root protrusion,gingival color,distal/middle gingi-val papilla of the study group were higher than those of the control group.There were no differences in swallowing function and masticatory function between the two groups before operation.After operation,the grades of swallowing function and masticatory function in the study group were higher than those in the control group.There were no differences in the incidences of postop-erative root fracture/coronal fracture,full crown shedding,and periapical lesions between the two groups,but the incidence of gingivitis in the study group was lower than that in the control group.Conclusion The results indicate that using glass fiber post and resin core to restore maxillary anterior tooth can achieve good long-term effect and safety while improving the patients’cosmetic effect,swallowing function
