摘 要:目的:探讨血清和支气管灌洗液中高迁移率族蛋白-1(HMGB-1)、鳞状细胞癌抗原(SCC)、神经元特异烯醇化酶(NSE)在老年肺癌患者中的表达及意义.方法:纳入2021年2月~2022年1月本院收治120例老年肺癌患者作为肺癌组,同时选择同期的120例良性肺病患者作为良性肺病组.比较肺癌组和良性肺病组血清和支气管灌洗液中HMGB-1、SCC、NSE水平,分析HMGB-1、SCC、NSE水平与患者肺癌分期和临床特征的关系,评估HMGB-1、SCC、NSE水平对Ⅲ、IV期肺癌的预测价值.结果:120例老年肺癌患者中,Ⅰ期、Ⅱ期、Ⅲ期、Ⅳ期分别有30、35、29、26例.肺癌组患者血清和支气管灌洗液中HMGB-1、SCC、NSE水平显著高于良性肺病组,且差异具有统计学意义.从I期~Ⅳ期血清和支气管灌洗液中HMGB-1、SCC、NSE水平都有升高的趋势.男性患者血清和支气管灌洗液中HMGB-1和NSE水平、血清SCC水平明显高于女性患者;发生远处组织转移的患者血清和支气管灌洗液中HMGB-1和NSE水平、支气管灌洗液SCC水平明显高于未发生远处组织转移的患者;小细胞型肺癌患者血清和支气管灌洗液中NSE水平明显高于非小细胞型肺癌患者.血清SCC的预测Ⅲ期、Ⅳ期期肺癌AUC值最高为0.851,灵敏度和特意度分别为0.708、0.945,最佳阈值为5.320ng/mL.结论:血清和支气管灌洗液中的HMGB-1、SCC、NSE水平与肺癌患者临床分期密切相关,血清SCC对Ⅲ期、Ⅳ期期肺癌预测能力最佳,可对临床提供一定的指导.Objective To investigate the expression and significance of high mobility group protein-1(HMGB-1),squa-mous cell carcinoma antigen(SCC)and neuron specific enolase(NSE)in serum and bronchial lavage fluid in elderly patients with lung cancer.Methods 120 elderly patients with lung cancer treated in our hospital from February 2021 to January 2022 were included as the lung cancer group,and 120 patients with benign lung disease in the same period were selected as the be-nign lung disease group.To compare the levels of HMGB-1,SCC and NSE in serum and bronchial lavage fluid between lung cancer group and benign lung disease group,analyze the relationship between the levels of HMGB-1,SCC and NSE and the stage and clinical characteristics of lung cancer,and evaluate the predictive value of HMGB-1,SCC and NSE in stage III and IV lung cancer.Results Among the 120 elderly patients with lung cancer,there were 30,35,29 and 26 patients with stage I,II,III and IV respectively.The levels of HMGB-1,SCC and NSE in serum and bronchial lavage fluid in lung cancer group were significantly higher than those in benign lung disease group.The levels of HMGB-1,SCC and NSE in serum and bronchial lav-age fluid increased from stage I to stage IV.The levels of HMGB-1,NSE and SCC in serum and bronchial lavage fluid of male patients were significantly higher than those of female patients;The levels of HMGB-1 and NSE in serum and bronchial lavage fluid and the level of SCC in bronchial lavage fluid in patients with distant tissue metastasis were significantly higher than those without distant tissue metastasis.The levels of NSE in serum and bronchial lavage fluid of patients with small cell lung cancer were significantly higher than those of patients with non-small cell lung cancer.The highest AUC value of serum SCC in pre-dicting stage III and IV lung cancer was 0.851,the sensitivity and specificity were 0.708 and 0.945 respectively,and the optimal threshold was 5.320ng/ml.Conclusion The levels of HMGB-1,SCC and NSE in serum and bronchial lavage f
