摘　　要：目的探讨客观结构化临床考试运用于腹膜透析患者培训的效果。方法将104例腹膜透析置管术患者随机分为对照组与观察组各52例。两组统一实施标准化培训,参加笔试和操作考核后对照组出院,观察组再参加客观结构化临床考试后出院。出院后随访6个月,比较两组笔试成绩、操作考核成绩、自我管理能力评分及腹膜炎发生情况。结果对照组47例、观察组50例完成研究。观察组出院后3个月及6个月自我管理能力量表评分、笔试成绩、腹透换液及出口护理考核评分显著高于对照组(均P<0.05),两组腹膜炎发生率及发生时间比较,差异无统计学意义(均P>0.05)。结论在腹膜透析患者培训中运用客观结构化临床考试,能提高患者的自我管理能力及操作成绩,但对腹膜炎的影响尚有待进一步研究。Objective To determine the effect of objective structured clinical examination(OSCE)in teaching of peritoneal dialysis(PD)patients.Methods A total of 104 PD patients were randomized into a control group and an observation group,with 52 in each.Both groups were given standardized teaching and they were asked to complete theoretical and technical tests before discharge.Patients in the observation group were additionally required to finish OSCE.All patients were followed up for 6 months and their theoretical and technical performance,self-management ability and incidence of peritonitis were compared between the two groups.Results Forty-seven patients in the control group and fifty in the observation group completed the study.Self-management ability at 3 and 6 months post-discharge,theoretical test score,the scores of fluid removal and exit site care in the observation group were significantly higher than those in the control group(P<0.05 for all).There were no significant differences in the occurrence and time to the first PD peritonitis episode between the two groups(P>0.05 for both).Conclusion Application of OSCE in teaching of PD patients can improve their self-management ability and technical performance.However,the effect on peritonitis needs further study.
