LED联合重组人酸性成纤维细胞生长因子和多黏菌素B软膏治疗老年难治性皮肤溃疡的疗效

摘　　要：目的观察发光二极管(light emitting diode,LED)红光联合重组人酸性成纤维细胞生长因子及复方多黏菌素B软膏治疗老年难治性皮肤溃疡患者的临床疗效及安全性。方法回顾性分析2018年2月至2020年10月,老年难治性皮肤溃疡患者58例的临床资料,按照治疗方法不同分为试验组和对照组,每组患者各29例。对照组给予常规外科清创换药,喷洒外用重组人酸性成纤维细胞表皮生长因子外用溶液后,外敷复方多黏菌素B软膏,早期换药频率为2~3次/d,创面好转后,换药频次改为间隔2~3 d换药1次;试验组在对照组的基础上联合LED照射,波长(633±10)nm,功率密度80~100 mW/cm^(2),照射时间20 min,照射频次与换药频次同步,早期频率为2~3次/d,创面好转后,改为间隔2~3 d照射1次,共治疗4周为一个疗程。一个疗程治疗后1周,测量两组患者溃疡面积大小,进行生活质量评分,判定临床疗效。并记录不良反应发生率。结果治疗前两组患者的溃疡面积及生活质量评分比较,差异均无统计学意义(P>0.05)。治疗后1周,试验组患者溃疡面积显著低于治疗前(P<0.05),生活质量评分显著高于治疗前(P<0.05);对照组患者治疗后溃疡面积及生活质量评分虽然较治疗前均有不同程度改善,但与治疗前比较,差异无统计学意义(P>0.05)。试验组总有效率显著高于对照组,差异具有统计学意义(P<0.05)。两组均未发生明显不良反应。结论LED红光联合重组人酸性成纤维细胞生长因子及复方多黏菌素B软膏治疗老年难治性皮肤溃疡患者疗效确切,安全性好。Objective To observe the advantages to deal chronic intractable skin ulcers in senile patients with a therapy that combines red LED light,rh-AFGF(recombinant human acidic fibroblast growth factor) and compound polymyxin B ointment.Methods A retrospective analysis was made on 58 senile patients of chronic intractable skin ulcers that were treated during the period from Feb.2018 to Oct.2020.They were divided into two groups by treating methods:the experimental group(29 patients)and control group(29 patients).The patients in the control group were given rh-AFGF and compound polymyxin B ointment for external application.Those in the experimental group were given red LED irradiation in addition.The power density used for the treatment was 80-100 mW/cm~2,and the irradiation time was 20 minutes.The ulcer size and life quality score before and after the treatment were compared between the two groups.The observation period lasted for 1 week after treament.Results There was no significant difference in ulcer size or life quality score between the two groups before the treatment(P>0.05).After the treatment,significant changes to ulcer size and life quality score were observed in the experimental group(P<0.05).As to the control group,the improvement in the field of ulcer size and life quality score was not obvious(P>0.05).The difference between the two groups in the improvement was statistically significant(P<0.05).The total effective rate of experimental group was significantly higher than that of the control group(P>0.05).No obvious adverse reaction occurred in two groups.Conclusions The therapy combining red LED light,rh-AFGF and compound polymyxin B ointment is effective and safe in treating chronic intractable skin ulcers in senile patients.
