新蒲饮联合奥美拉唑治疗幽门螺杆菌相关性胃炎实验研究

摘　　要：目的探讨新蒲饮联合奥美拉唑治疗幽门螺杆菌相关性胃炎疗效的基础研究。方法将50只C57BL/6(B6)小鼠采用细菌灌胃的方法建立小鼠幽门螺杆菌(helicobacter pylori,Hp)相关性胃炎模型,造模成功后随机分为对照组、模型组、单纯新蒲饮组(新蒲饮组)、新蒲饮联合奥美拉唑组(新奥组)和阿莫西林+克拉霉素+奥美拉唑肠溶片组(胃三联组),后各给药组连续给药14 d。通过快速尿素酶法及吉姆萨染色观察Hp的分布及定植情况,采用酶联免疫吸附测定(enzyme linked immunosorbent assay,ELISA)检测血清白细胞介素-8(interleukin-8,IL-8)的表达,免疫组化以及蛋白质印迹法检测IL-8、核因子-κB(nuclear factor-k-gene binding,NF-κB)的蛋白表达,实时荧光定量PCR(quantificational real-time PCR,qRT-PCR)检测IL-8、NF-κB的信使RNA(message RNA,mRNA)表达。结果通过尿素酶检测及吉姆萨染色可以发现,与模型组比较,各给药组小鼠胃黏膜Hp明显减少,胃三联组与新奥组较为明显,新蒲饮组次之;治疗前,模型组IL-8、NF-κB蛋白和mRNA表达水平及血清IL-8含量均高于对照组(P<0.05);治疗后,与模型组比较,新蒲饮组、新奥组及胃三联组IL-8、NF-κB蛋白和mRNA表达水平及血清IL-8含量均明显降低(P<0.05);进一步分析发现,新奥组和胃三联组IL-8、NF-κB及血清IL-8含量明显低于新蒲饮组。结论动物实验表明新蒲饮能够治疗Hp相关性胃炎,联合奥美拉唑效果更优,与胃三联效果相似。由此可见,新蒲饮联合奥美拉唑有望成为治疗Hp相关性胃炎的新方法。Objective To investigate the basic research of Xinpu Yin(新蒲饮)combined with omeprazole in the treatment of Helicobacter pylori-associated gastritis.Methods A total of 50 C57BL/6(B6)mice were established by bacterial gavage.The models were randomly divided into control group,model group and Xinpu Yin group,Xinpu Yin combined with omeprazole group(Xin’ao group)and amoxicillin+clarithromycin+omeprazole enteric-coated tablets group(gastritis triple group),and each administration group was continuously administered for 14 days.The distribution and colonization of Helicobacter pylori(Hp)were observed by rapid urease method and Giemsa staining.The expression of serum interleukin-8(IL-8)was detected by enzyme linked immunosorbent assay(ELISA).The proteins of IL-8 and nuclear factor-κ-gene binding(NF-κB)were detected by immunohistochemistry and Western Blot.The message RNA(m RNA)expression of IL-8 and NF-κB were detected by quantificational real-time PCR(qRT-PCR).Results By urease test and Giemsa staining,it was found that compared with the model group,the Hp of the gastric mucosa of the mice in each group was significantly reduced,and the gastritis triple group and the Xin’ao group were more obvious,and the Xinpu Yin group was the second;IL-8,NF-κB protein and m RNA expression levels and serum IL-8 levels in the model group were higher than those in the control group(P<0.05).After treatment,compared with the model group,Xinpu Yin group,Xin’ao group and gastritis triple group IL-8,NF-κB protein and m RNA expression levels and serum IL-8 levels were significantly decreased in the triple group(P<0.05).Further analysis found that the levels of IL-8,NF-κB and serum IL-8 in Xin’ao group and gastritis triple group were significantly lower than those in Xinpu Yin group.Conclusion Animal experiments have shown that Xinpu Yin can treat Hp-associated gastritis,and the effect of Xinpu Yin combined omeprazole is better,similar to the effect of gastric triple.It can be seen that Xinpu Yin is expected to replace anti
