摘 要:目的:探讨生物膜联合羟基磷灰石生物陶瓷治疗根尖骨缺损的临床安全性与有效性。方法:选取根尖骨缺损患者86例作为研究对象,根据1∶1随机信封分配原则把患者分为联合组与对照组各43例,对照组给予羟基磷灰石生物陶瓷治疗,联合组给予生物膜联合羟基磷灰石生物陶瓷治疗,记录与随访临床安全性与有效性。结果:联合组治疗后12、24、48 h的疼痛视觉模拟量表(VAS)评分均低于对照组(均P<0.05)。两组治疗后3个月的旧根尖周指数(O-PAI)评分低于治疗前(P<0.05),联合组低于对照组(P<0.05)。联合组治疗后3个月的愈合率为97.7%,高于对照组的83.7%(P<0.05)。联合组治疗后3个月的红肿、窦道、脓肿等并发症发生率为4.7%,低于对照组的20.9%(P<0.05)。两组治疗后3个月的P物质含量高于治疗前(P<0.05),联合组高于对照组(P<0.05)。结论:生物膜联合羟基磷灰石生物陶瓷治疗根尖骨缺损能促进缓解疼痛,改善患者的临床症状,还可提高治疗愈合率,可提高血清P物质含量,减少并发症的发生。Objective:To investigate the clinical efficacy and safety of biofilm combined with hydroxyapatite bioceramics in the treatment of apical bone defects.Methods:A total of 86 cases of patients with apical bone defects were selected as the research subjects.All the cases were divided into combination group and control group of 43 cases in each groups accorded to the principle of 1 to 1 random envelope allocation.The control group was given hydroxyapatite bioceramic treatment,and the combination group was given biofilm combined with hydroxyapatite bioceramic treatment.The clinical efficacy and safety were recorded and followed up.Results:The VAS scores of the combination group at 12,24 and 48 hours after treatment were lower than those of the control group(all P<0.05).The O-PAI scores of the two groups at 3 months after treatment were higher than those before treatment,and the combination group was higher than the control group(all P<0.05).The healing rate after 3 months of treatment in the combination group were 97.7%,which were higher than 83.7%in the control group(P<0.05).The incidence of complications such as swelling,sinus and abscess in the combination group was 4.7%after 3 months of treatment,which was lower than 20.9%in the control group(P<0.05).The content of substance P in the two groups at 3 months after treatment was higher than that before treatment,and the combination group was higher than the control group(all P<0.05).Conclusion:The combination of biofilm and hydroxyapatite bioceramics in the treatment of apical bone defects can promote pain relief,improve the clinical symptoms of patients,increase the healing rate,increase the content of serum substance P,and reduce the occurrence of complications.
