益气聪明汤联合盐酸多奈哌齐片对气虚型卒中后认知障碍患者认知功能及星形胶质源性蛋白的影响

摘　　要：目的观察益气聪明汤联合盐酸多奈哌齐片对气虚型卒中后认知障碍(PSCI)患者认知功能及星形胶质源性蛋白(S100β)的影响。方法将81例气虚型PSCI患者按照随机数字表法分为2组,对照组40例在常规治疗基础上口服盐酸多奈哌齐片治疗,治疗组41例在对照组治疗基础上联合口服益气聪明汤治疗。2组疗程为4周。比较2组治疗前后简易智能精神状态量表(MMSE)各项评分、日常生活能力量表(ADL)评分、美国国立卫生研究院神经功能缺损评分(NIHSS)、气虚证候评分及血清S100β水平,并统计2组疗效。结果治疗组总有效率73.17%(30/41),对照组总有效率60.00%(24/40),治疗组疗效优于对照组(P<0.05)。2组治疗后MMSE各单项评分及总分均较本组治疗前升高(P<0.05),且治疗组治疗后均高于对照组(P<0.05)。2组治疗后ADL评分均较本组治疗前升高(P<0.05),且治疗组治疗后高于对照组(P<0.05)。2组治疗后NIHSS均较本组治疗前降低(P<0.05),且治疗组治疗后低于对照组(P<0.05)。治疗组治疗后气虚证候各项评分均较本组治疗前降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05)。2组治疗后血清S100β水平均较本组治疗前降低(P<0.05),且治疗组治疗后低于对照组(P<0.05)。结论益气聪明汤联合盐酸多奈哌齐片治疗可有效改善气虚型PSCI患者气虚证候,减轻脑损伤及神经功能缺损,改善认知功能,进而提高日常生活能力。Objective To observe the impact of Yiqi Congming Decoction with donepezil hydrochloride tablets on cognitive function and astrocyte-derived protein(S100β)of patients with post-stroke cognitive impairment(PSCI)of qi deficiency.Methods Totally 81 patients with PSCI(qi deficiency)were randomly assigned into treatment group(n=41)and control group(n=40),apart from conventional treatment,all patients were managed by oral donepezil,and those in treatment group were additionally treated with Yiqi Congming Decoction.Treated for 4 weeks,aiming to compare Mini-Mental State Examination(MMSE),activity of daily living(ADL)scale,National Institutes of Health Stroke Scale(NIHSS),Qi deficiency syndrome score,serum S100βlevel;the curative effect calculated.Results The overall effective rate was 73.17%(30/41)in the treatment group,higher than that of 60.00%(24/40)in the control group(P<0.05).After treatment,MMSE scores in groups were increased(P<0.05),which increased in the treatment group when compared with the control group(P<0.05);ADL scores in groups were increased(P<0.05),which in the treatment group were more common(P<0.05);NIHSS in groups was decreased(P<0.05),and the improvement in the treatment group was more obvious(P<0.05);qi deficiency syndrome score in the treatment group was decreased(P<0.05),which in the treatment group was more lower(P<0.05);serum S100βlevel in groups was decreased(P<0.05),the treatment group was more prevalent(P<0.05).Conclusion For patients with PSCI(qi deficiency),Yiqi Congming Decoction with donepezil hydrochloride tablets can relieve qi deficiency syndrome,it not only improves cognitive function and daily living ability,but also reduces brain injury and neurologic impairment.
