养阴解毒汤联合放化疗治疗气阴两虚毒热蕴肺型鼻咽癌晚期疗效观察

摘　　要：目的观察养阴解毒汤联合放化疗治疗气阴两虚、毒热蕴肺型鼻咽癌晚期患者的临床疗效。方法选取河北省廊坊市第四人民医院耳鼻喉科就诊的鼻咽癌晚期住院患者98例,按照随机数字表法分为2组,各49例。对照组仅予放化疗措施;观察组于放化疗第1天采取养阴解毒汤内服。连续治疗9周。比较2组临床疗效、中医证候评分、治疗期间不良反应、生活质量评分。结果观察组总有效率85.71%(42/49),对照组总有效率65.31%(32/49),治疗组疗效优于对照组(P<0.05)。与本组治疗前比较,对照组治疗后中医证候各项评分均增加(P<0.05),且观察组治疗后中医证侯各项评分均低于对照组治疗后(P<0.05)。治疗过程中观察组白细胞下降、中性粒细胞减少、恶心呕吐、放射性皮炎、口腔内膜炎发生率均低于对照组(P<0.05)。观察组治疗前后中文版头颈部癌患者生命质量量表(FACT-H&N)评分比较差异无统计学意义(P>0.05);对照组治疗后FACT-H&N评分较治疗前明显增加(P<0.05),且观察组治疗后低于对照组(P<0.05)。结论养阴解毒汤联合放化疗治疗气阴两虚、毒热蕴肺型鼻咽癌晚期患者,可提高治疗效果,防止放化疗对中医证候和生存质量产生进一步损害,减轻放化疗所致毒副反应。Objective To observe the clinical efficacy of Yangyin Jiedu Decoction combined with radiotherapy and chemotherapy on patients with advanced nasopharyngeal carcinoma(NPC)of qi and yin deficiency and toxic heat accumulating lung.Methods Ninety-eight patients with advanced NPC admitted to Department of Otolaryngology,Fourth People's Hospital of Langfang were selected and divided into two groups according to random number table method including 49 patients in each group.The control group only received radiotherapy and chemotherapy,and the observation group orally took Yangyin Jiedu Decoction on the first day of chemoradiotherapy.After 9 weeks'treatment,the clinical efficacy,traditional Chinese medicine(TCM)syndrome scores,adverse reactions during treatment,and score of life quality of the two groups were compared.Results The overall effective rate of the observation group was 85.71%(42/49),which was significantly higher than 65.31%(32/49)of the control group(P<0.05).Compared with before treatment,the TCM syndrome scores in the control group increased after treatment(P<0.05),and which were lower in the observation group than in the control group after treatment(P<0.05).During the treatment,the incidence of leukopenia,neutropenia,nausea and vomiting,oral mucositis and radioactive dermatitis in the observation group were significantly lower than those in the control group(P<0.05).The difference was not statistically significant in the FACT-H&N score in the observation group before and after treatment(P>0.05).After treatment,FACT-H&N score in the control group was significantly increased(P<0.05),and the observation group was significantly lower than the control group(P<0.05).Conclusion For patients with advanced NPC(qi and yin deficiency and toxic heat accumulating lung cases),Yangyin Jiedu Decoction can improve the therapeutic effect,prevent further damage and reduce the toxic and side effects caused by radiotherapy and chemotherapy.
