双侧上肢训练对脑卒中早期患者上肢功能障碍的疗效

摘　　要：目的探讨双侧上肢训练对脑卒中早期患者上肢运动功能障碍的疗效。方法将60例脑卒中偏瘫患者随机分成单侧组和双侧组,每组30例。单侧组采用传统的患侧上肢运动功能训练,双侧组采用双侧上肢运动功能训练。两组干预时间均为30 min/次,1次/d,5 d/周,共4周。分别于干预前后采用Fugl-Meyer运动功能评定的上肢部分(FMA-UE)、患侧握力及改良Barthel指数(MBI)进行评价。结果干预前,双侧组及单侧组患者FMA-UE评分、患侧握力、MBI评分比较差异无统计学意义(均P>0.05);干预后,双侧组及单侧组FMA-UE评分、患侧握力、MBI评分均较干预前显著升高(均P<0.05)。双侧组干预后FMA-UE评分、患侧握力、MBI评分显著高于单侧组(均P<0.05)。结论与传统患侧上肢训练比较,双侧上肢训练对早期脑卒中上肢功能障碍患者的上肢运动功能及日常生活活动能力有更积极的影响,值得推广使用。Objective To investigate the effect of bilateral upper limb training on upper limb motor dysfunction in early stroke patients.Methods Sixty stroke patients with hemiplegia were randomly divided into unilateral group and bilateral group,with 30 patients in each group.The unilateral group received traditional upper limb motor function training,and the bilateral group received bilateral upper limb motor function training.The intervention time of both groups was 30 min each time,once a day,5 d for a week and 4 weeks in total.The upper-extremity section of Fugl-Meyer assessment(FMA-UE),grip strength of the affected side and modified barthel index(MBI)were compared before and after intervention.Results Before intervention,there was no significant difference in FMA-UE score,grip strength of the hemiplegic side and MBI score between two groups(all P>0.05).After intervention,FMA-UE score,grip strength of the hemiplegic side and MBI score in both bilateral and unilateral groups were significantly higher than those before intervention(all P<0.05).After intervention,FMA-UE score,grip strength of the hemiplegic side and MBI score in bilateral group were significantly higher than those in unilateral group(all P<0.05).Conclusion Compared with traditional upper limb training on the affected side,bilateral upper limb training has a more positive effect on the upper limb motor function and the ability of daily living activities in patients with early stroke upper limb dysfunction,which is worthy of popularization.
