摘 要:目的探讨甲状腺癌组织蛋白激酶B(protein kinase B,Akt)/细胞周期素D1(cyclin D1,CCND1)通路相关蛋白表达及其与临床病理特征的关系。方法选取2018年10月至2020年6月宿迁市第一人民医院收治的86例甲状腺癌患者,收集患者在根治术治疗中切除的甲状腺癌组织及配对癌旁组织。检测甲状腺癌组织及配对癌旁组织Akt、磷酸化蛋白激酶B(phosphorylation Akt,p-Akt)、CCND1及周期蛋白依赖性激酶4(cyclin-dependent kinase 4,CDK4)蛋白表达水平,分析其与临床病理特征的关系。结果与癌旁组织相比,甲状腺癌组织Akt、CCND1、p-Akt、CDK4蛋白阳性表达率均显著升高,差异有统计学意义[(3.49%比77.91%),(2.33%比72.09%),(2.33%比68.61%),(1.16%比60.47%)χ^(2)值分别为98.67、89.58、82.53、70.93,P值均<0.001]。Ⅲ~Ⅳ期甲状腺癌组织Akt、CCND1、p-Akt及CDK4蛋白表达阳性率显著高于Ⅰ~Ⅱ期组织(χ^(2)值分别为6.03、7.96、5.01、6.45,P值均<0.05),低分化甲状腺癌组织Akt、CCND1、p-Akt及CDK4蛋白表达阳性率显著高于中/高分化组织(χ^(2)值分别为4.85、7.41、6.17、4.67,P值均<0.05),淋巴结转移甲状腺癌组织Akt、CCND1、p-Akt及CDK4蛋白表达阳性率显著高于淋巴结未转移组织(χ^(2)值分别为6.61、15.07、10.10、4.81,P值均<0.05),病理分型分布差异显著(χ^(2)值分别为11.30、9.35、8.52、13.87,P值均<0.05)。结论甲状腺癌患者癌组织Akt、CCND1、p-Akt、CDK4蛋白阳性表达率高于癌旁组织,且与临床分期、分化程度、病理分型及淋巴结转移密切相关。Objective To investigate the expression of protein kinase B(Akt)/cyclin D1(CCND1)pathway related proteins in thyroid carcinoma and its relationship with clinicopathological features.Methods Total of 86 patients with thyroid cancer in Suqian First People's Hospital from October 2018 to June 2020 were selected.The thyroid cancer tissues and paired adjacent tissues removed during radical surgery were collected.The expression levels of Akt,phosphorylation Akt(p-Akt),CCND1 and cyclin-dependent kinase 4(CDK4)in thyroid carcinoma tissues and paired adjacent tissues were detected,and their relationships with clinicopathological features were analyzed.Results Compared with the adjacent tissues,the positive expression rates of Akt,CCND1,p-Akt and CDK4 in thyroid carcinoma tissues were significantly increased[(3.49%vs 77.91%),(2.33%vs 72.09%),(2.33%vs 68.61%),(1.16%vs 60.47%),χ^(2) values were 98.67,89.58,82.53 and 70.93 respectively,all P values<0.001].The positive rates of Akt,CCND1,p-Akt and CDK4 protein expression in stageⅢ~Ⅳthyroid cancer tissues were significantly higher than those in stageⅠ~Ⅱtissues(χ^(2) values were 6.03,7.96,5.01 and 6.45,all P values<0.05).The positive rates of Akt,CCND1,p-Akt and CDK4 protein expression in poorly differentiated thyroid cancer tissues were significantly higher than those in moderately/highly differentiated tissues(χ^(2) values were 4.85,7.41,6.17 and 4.67,all P values<0.05).The positive rates of Akt,CCND1,p-Akt and CDK4 protein expression in lymph node metastatic thyroid cancer tissues were significantly higher than those in lymph node non metastatic tissues(χ^(2) values were 6.61,15.07,10.10 and 4.81,all P values<0.05).The distribution of pathological types was significantly different(χ^(2) values were 11.30,9.35,8.52 and 13.87,all P values<0.05).Conclusion The positive expression rates of Akt,CCND1,p-Akt and CDK4 proteins in thyroid cancer tissues were higher than those in adjacent tissues,and they were closely related to clinical stage,differentiation degree,pathol
