摘　　要：目的探究肺腺癌患者表皮生长因子受体(EGFR)突变与CT特征的相关性。方法搜集就诊于本院经病理证实为肺腺癌的77例患者。所有受试者都接受了EGFR突变状态检测,并在接受治疗前均行胸部CT检查,由两名医师评价肺部CT特征,使用卡方检验、Fisher确切概率法比较肺部CT特征、临床特征与EGFR突变状态的关系,选出有统计学差异的变量进行二元Logistic回归分析来确定预测EGFR突变的独立危险因素。结果77例肺腺癌患者中,EGFR突变型39例,野生型38例,EGFR突变在女性、无吸烟史、较少淋巴结转移、少有胸腔积液/心包积液的患者中更常见,并且胸部CT常表现为有空气支气管征和小血管穿行的实性密度病灶。Logistic回归分析显示女性、空气支气管征和无胸腔/心包积液与EGFR突变相关。结论在肺腺癌患者中,恶性胸腔/心包积液较少和CT有空气支气管征的女性患者更可能提示存在EGFR基因突变。Objective To study the correlation between EGFR mutation and CT features in patients with lung adenocarcinoma.Methods 77 patients with lung adenocarcinoma confirmed pathologically in our hospital were selected.All subjects received EGFR mutation status testing,and had chest CT examinations before receiving treatment.Two physicians evaluated lung CT features,compared lung CT features and clinical features using Chi-square test and Fisher exact probability method.With regard to the relationship between EGFR mutation status,the variables with statistical differences were selected for binary logistic regression analysis to determine the independent risk factors for predicting EGFR mutation.Results Among 77 patients with lung adenocarcinoma,39 were EGFR mutant and 38 were wild-type.EGFR mutations were more common in women with no history of smoking,fewer lymph node metastases,and fewer pleural effusions/pericardial effusions,and chest CT often manifested as solid density lesions with air bronchial signs and small blood vessels.Conclusion Among patients with lung adenocarcinoma,women with less malignant pleural/pericardial effusion and CT with air bronchial signs are more likely to predict the presence of EGFR gene mutations.
