摘　　要：目的探讨丁苯酞软胶囊对急性缺血性脑卒中患者血液流变学、血流动力学指标的影响。方法选取2019年1月至2020年12月陕西省核工业二一五医院神经内科收治的100例急性缺血性脑卒中患者作为研究对象,按照随机数字表法将其分为对照组(50例,常规治疗)和观察组(50例,丁苯酞软胶囊+常规治疗)。比较两组治疗前、后的血液流变学指标、血流动力学指标、血清炎症因子水平、美国国立卫生研究院卒中量表(NIHSS)、日常生活能力量表(ADL)、WHO生存质量测定量表简表(WHOQOL-BREF)评分。结果治疗后,观察组的全血高切黏度、全血低切黏度、血浆黏度均低于对照组(P<0.05)。治疗后,两组大脑中动脉(MCA)、大脑前动脉(ACA)、大脑后动脉(PCA)收缩期的血液峰值流速均升高,且观察组高于对照组(P<0.05)。治疗后,两组的血清白细胞介素-6(IL-6)、C反应蛋白(CRP)、降钙素原(PCT)水平均降低,且观察组低于对照组(P<0.05)。治疗后,两组的NIHSS评分均降低,ADL、WHOQOL-BREF评分均升高,且观察组优于对照组(P<0.05)。结论丁苯酞软胶囊治疗急性缺血性脑卒中患者不仅能促进血液流变学、血流动力学指标改善,减轻炎症反应,还能保护神经功能,利于患者日常生活能力及生活质量的提升,值得推广。Objective To investigate the effects of butylphthalide soft capsule on hemorheological and hemodynamic indexes in patients with acute ischemic stroke.Methods A total of 100 patients with acute ischemic stroke admitted in the neurology department of No.215 Hospital of Shaanxi Nuclear Industry from January 2019 to December 2020 were selected as the research objects,and the patients were divided into control group(50 cases,conventional treatment)and observation group(50 cases,butylphthalide soft capsule+conventional treatment)according to the random number table method.The hemorheological indexes,hemodynamic indexes,serum inflammatory factors levels,national institute of health stroke scale(NIHSS),activities of daily living scale(ADL)and WHO quality of life-BREF(WHOQOL-BREF)scores were compared between the two groups before and after treatment.Results After treatment,the whole blood high shear viscosity,whole blood low shear viscosity and plasma viscosity in the observation group were lower than the control group(P<0.05).After treatment,the peak systolic blood flow rates of middle cerebral artery(MCA),anterior cerebral artery(ACA)and posterior cerebral artery(PCA)in the two groups increased,and those in the observation group were higher than the control group(P<0.05).After treatment,the levels of serum interleukin-6(IL-6),C-reactive protein(CRP)and procalcitonin(PCT)in the two groups decreased,and those in the observation group were lower than the control group(P<0.05).After treatment,NIHSS score in the two groups decreased,ADL and WHOQOL-BREF scores increased,and those in the observation group were better than the control group(P<0.05).Conclusion The acute ischemic stroke patients treated with butylphthalide soft capsule can not only promote the improvement of hemorheological and hemodynamic indexes,reduce the inflammatory reaction,but also protect the neural function,which is conducive to the improvement of patients'ability of daily living and quality of life,and is worthy of promotion.
