摘 要:目的探讨术中超声辅助显微切除术治疗脑胶质瘤的临床效果。方法收集2015年1月至2018年1月河池市人民医院收治的113例脑胶质瘤患者的临床资料,根据是否接受术中超声辅助手术将患者分为超声组(术中超声辅助显微切除术,58例)和常规组(常规显微切除术,55例)。对比2组患者的临床手术资料、卡氏功能状态(KPS)评分、改良Barthel指数评分以及远期疗效。结果超声组患者肿瘤暴露时间明显短于常规组(P<0.05);超声组患者肿瘤切除程度明显高于常规组,而术后脑水肿程度则低于常规组(P<0.05);超声组患者术后6个月时的KPS评分及改良Barthel指数评分均高于常规组(P<0.05);超声组患者3年生存率均高于常规组,而3年复发率则低于常规组(P<0.05)。结论术中超声辅助显微切除术能缩短肿瘤暴露时间,提高肿瘤切除程度,保护神经功能和改善生存预后。Objective To investigate the clinical effect of intraoperative ultrasound-assisted microresection in treatment of glioma.Methods The clinical data of 113 patients with glioma treated in Hechi People’s Hospital from January 2015 to January 2018 were collected for study.By depending on whether or not they received intraoperative ultrasound-assisted microresection,they were divided into the ultrasound group(58 cases)and the conventional group(55 cases).Clinical surgical data,KPS scores,the modified Barthel indexes and long-term efficacy were compared between the two groups.Results The tumor exposure time of the ultrasound group was significantly less than that of the conventional group(P<0.05).The extent of tumor resection in the ultrasound group was significantly higher than that in the conventional group,while the seriousness of postoperative brain edema in the former was less severe than that in the latter(P<0.05).The KPS scores and modified Barthel indexes at month 6 after surgery in the ultrasound group were all higher than those in the conventional group at the same time point(P<0.05).The 3-year survival rate of the ultrasound group was higher than that of the conventional group,and the 3-year recurrence rate of the former was lower than that of the latter(P<0.05).Conclusion Intraoperative ultrasonic-assisted microscopic resection could shorten tumor exposure time,improve the extent of tumor resection,protect nerve function and improve survival and prognosis.
