摘 要:目的探讨扩散加权成像(DWI)及增强T_(1) mapping在评估肺癌病理类型中的应用价值.方法收集58例经病理确诊为肺癌患者的MRI资料,分为鳞癌组17例、腺癌组26例、小细胞癌组15例.所有患者均进行胸部DWI、T_(1) mapping序列扫描,分别于增强前、后采集T_(1) mapping图像.测量肿瘤的表观扩散系数(ADC)值,增强前、后T_(1)值以及计算T_(1)值的下降率(ΔT_(1)),并比较3组之间的差异.结果3组的增强后T_(1)值和ΔT_(1)差异均有统计学意义(P<0.05);小细胞癌组的增强后T_(1)值高于腺癌组、鳞癌组,ΔT_(1)低于腺癌组、鳞癌组;腺癌组的增强后T_(1)值低于鳞癌组、小细胞癌组,ΔT_(1)高于鳞癌组、小细胞癌组(P<0.05);3组ADC值、增强前T_(1)值差异均无统计学意义(P>0.05).结论增强后T_(1)值对不同病理类型肺癌的诊断效能优于ADC值,增强T_(1) mapping为肺癌的早期诊断提供客观依据.Objective To explore the application value of diffusion weighted imaging(DWI)and enhanced T_(1) mapping in evaluating pathological types of lung cancer.Methods MRI data of 58 pathologically confirmed lung cancer patients were collected and divided into three groups:squamous cell carcinoma(17 cases),adenocarcinoma(26 cases)and small cell cancer(15 cases).All patients underwent chest DWI and T_(1) mapping sequences.T_(1) mapping was performed before and after enhancement.Apparent diffusion coefficients(ADC),pre and post-Tr value,and the reduction rate of T_(1) value(△T_(1))were calculated,then their differences in the three groups were compared.Results The enhanced T_(1) value and △T_(1)differences in the three groups were statistically significant(P<0.05).The enhanced T_(1) value of small cell cancer was higher than that of adenocarcinoma and squamous cell carcinoma,while △T_(1) was lower than adenocarcinoma and squamous cell carcinoma.The enhanced T_(1) value of adenocarcinoma was lower than that of squamous cell carcinoma and small cell cancer,while OT was higher than squamous cell carcinoma and small cell cancer(P<<0.05).There were no significant differences of ADC value and native T_(1) value in three groups(P>0.05).Conclusion The diagnostic efficacy of enhanced T_(1) value is higher than that of ADC in different pathological types of lung cancer.Enhanced T_(1) mapping provides an objective basis for early diagnosis of lung cancer.
