摘 要:目的:探析核磁共振弥散加权成像(MRI-DWI)联合磁敏感加权成像(SWI)在急性缺血性脑卒中的应用价值。方法:收集2020年1月-2021年1月牡丹江医学院第二附属医院收治的急性缺血性脑卒中患者80例临床资料,均经脑部CT检查确诊,并行MRI-DWI、SWI,探析MRI-DWI联合SWI在急性缺血性脑卒中的应用效果。比较急性缺血性脑卒中患者病灶检出情况,MRI-DWI联合SWI检查急性脑缺血患者病灶分布部位情况。结果:以经脑部CT结果作为“金标准”,80例急性缺血性脑卒中共检出病灶95个;MRI-DWI联合SWI检查急性缺血性脑卒中患者病灶部位的检出率高于MRI-DWI、SWI单独检出率,差异有统计学意义(P<0.05)。经脑部CT确诊,95个病灶组织中皮质/皮质下出血37例,基底节区27例,丘脑22例,小脑15例;MRI-DWI联合SWI检查急性脑缺血患者病灶部位与脑部CT检查结果,具有极好的一致性(Kappa=0.918,P<0.001)。结论:MRI-DWI联合SWI可有效提高急性缺血性脑卒中病灶检出率,明确病灶分布情况。Objective:To explore the application value of magnetic resonance diffusion-weighted imaging(MRI-DWI)combined with susceptibility-weighted imaging(SWI)in acute ischemic stroke.Methods:The clinical data of 80 patients with acute ischemic stroke who were admitted to the Second Affiliated Hospital of Mudanjiang Medical University from January 2020 to January 2021 were collected,all of the patients were diagnosed by brain CT examination,and MRI-DWI and SWI were performed.Application effect of MRI-DWI combined with SWI in acute ischemic stroke was explored.The detection of lesions in patients with acute ischemic stroke was compared,and the distribution of lesions in patients with acute cerebral ischemia was examined by MRI-DWI combined with SWI.Results:Using the results of brain CT as the"gold standard",a total of 95 lesions were detected in 80 patients with acute ischemic stroke.The lesion detection rate of MRI-DWI combined with SWI in patients with acute ischemic stroke was higher than that of MRI-DWI and SWI alone,and the difference was statistically significant(P<0.05).Diagnosed by brain CT,there were 37 cases of cortical/subcortical hemorrhage in 95 lesions,27 cases of basal ganglia,22 cases of thalamus,and 15 cases of cerebellum;the results of MRI-DWI combined with SWI to examine the lesions in patients with acute cerebral ischemia were well consistent with and brain CT examination results(Kappa=0.918,P<0.001).Conclusion:MRI-DWI combined with SWI can effectively improve the detection rate of acute ischemic stroke lesions and clarify the distribution of lesions.
