摘　　要：盆底肌肌肉紧张性疼痛是前列腺痛的一组常见证候,有部分学者指出前列腺痛本质是慢性前列腺炎引起的一种盆底肌肉及尿道括约肌的痉挛性疼痛。经方中“虚劳里急”主要以形体消瘦、腹部肌肉紧张、拘急时痛为临床表现,“里急”是指发生在少腹部的肌肉紧张、拘急时痛的症状表现,“虚劳”是指因脏腑亏损、气血阴阳虚衰、久病难复引起的多种慢性虚弱性疾病证候的总称。前列腺痛属于慢性盆腔疼痛综合征,与阴阳气血衰弱、脾失健运、肝气不舒、血瘀脉络等有着紧密联系。本文探讨少腹部及盆底肌肌肉紧张为表现的前列腺痛,从探讨“虚劳里急”条文及前列腺痛的病因病机出发,并在前列腺痛的治法与方药方面,着重剖析“温中补气,和里缓急”的黄芪建中汤在治疗肌肉紧张性前列腺痛的作用机制及应用。Pelvic floor muscle tension pain is a group of common symptoms of prostatic pain.Some scholars point out that prostatic pain is essentially a spastic pain of pelvic floor muscles and urethral sphincter caused by chronic prostatitis.In the classic prescription,"Xulao Liji"mainly takes physical emaciation,abdominal muscle tension and pain during acute pain as the clinical manifestations."Liji"refers to the symptoms of muscle tension and pain during acute pain in the lower abdomen."Xulao"refers to the general name of a variety of chronic debilitating disease syndromes caused by internal loss,deficiency of Qi,blood,Yin and Yang,and difficulty in recovering from a long illness.Prostatic pain is a chronic pelvic pain syndrome,which is closely related to the weakness of Yin and Yang Qi and blood,the loss of healthy movement of spleen,depression of liver Qi and so on.This paper discusses prostatic pain manifested by lower abdomen and pelvic floor muscle tension,starting from the discussion of the articles of"Xulao Liji"and the etiology and pathogenesis of prostatic pain,and in the treatment and prescription of prostate pain,this paper focuses on the mechanism and application of Huangqi Jianzhong decoction in the treatment of muscle tension prostate pain.
