摘　　要：目的分析肾透明细胞癌中KIF4A的表达及其对患者术后生存的预测价值。方法选取接受根治性肾切除术的肾透明细胞癌患者70例,术中获取癌组织和癌旁组织(距离肿瘤3 cm),检测KIF4A的表达。查看患者病案信息,统计患者的性别、年龄、临床分期、Fuhrman核分级、肿瘤大小、淋巴结转移和远处转移情况。随访60个月,统计患者生存时间。分析癌组织和癌旁组织KIF4A阳性表达情况,KIF4A表达与临床病理特征的关系,KIF4A表达对预后的影响。结果肾透明细胞癌组织KIF4A阳性率高于癌旁组织(P<0.05)。KIF4A表达与年龄、临床分期、Fuhrman核分级、肿瘤大小、淋巴结转移和远处转移有关(P<0.05),与性别无关(P>0.05)。KIF4A阴性表达患者平均生存时间为(47.69±6.02)个月,阳性表达患者平均生存时间为(30.59±4.05)个月,差异有统计学意义(P<0.05)。KIF4A阳性表达与生存时间呈负相关。结论肾透明细胞癌中KIF4A阳性表达影响术后生存时间,可作为预测术后预后的指标。Objective To investigate the expression of KIF4A in clear-cell renal cell carcinoma and its predictive value for postoperative survival time.Methods 70 patients with clear-cell renal cell carcinoma underwent radical nephrectomy were enrolled.The expression of KIF4A in tumor tissues and adjacent normal tissues(3cm from tumor)were detected.Data including gender,age,clinical stage,Fuhrman nuclear grade,tumor size,lymph node metastasis and distant metastasis were collected by medical records.Patients were followed up for 60 months and the survival time was counted.The positive expression of KIF4A in cancer and adjacent normal tissues were analyzed.Then the relationship between KIF4A expression and clinicopathological features were analyzed,and the impact of KIF4A expression on prognosis was explored.Results The positive rate of KIF4A in clear-cell renal cell carcinoma tissues was higher than that in adjacent normal tissues(P<0.05).The expression of KIF4A was correlated with the age,clinical stage,Fuhrman nuclear grade,tumor size,lymph node metastasis and distant metastasis(P<0.05),while had no correlation with the gender(P>0.05).The mean survival time was(47.69±6.02)months for patients with negative KIF4A expression and(30.59±4.05)months for patients with positive KIF4A expression,with statistical difference(P<0.05).The positive expression of KIF4A was negatively correlated with the postoperative prognostic time.Conclusion The positive expression of KIF4A affects postoperative survival time,so it can be used as a predictor of postoperative prognosis.
