Gal-1、GPX3及Tg水平与甲状腺癌病情严重程度的关系分析

摘　　要：目的分析半乳糖凝集素-1(Gal-1)、谷胱甘肽过氧化物酶3(GPX3)及甲状腺球蛋白(Tg)水平与甲状腺癌病情严重程度的关系。方法选取70例甲状腺癌患者,另选择同期就诊的70例甲状腺良性病变患者作为对照,两组均进行血清Gal-1、GPX3、Tg水平检测,对比其血清Gal-1、GPX3、Tg水平,评估Gal-1、GPX3、Tg对甲状腺癌的诊断价值,分析甲状腺癌患者临床参数与Gal-1、GPX3及Tg水平的关系。结果甲状腺癌组血清Gal-1、Tg水平高于甲状腺良性病变组(P<0.05),GPX3水平低于甲状腺良性病变组(P<0.05)。当血清Gal-1、GPX3、Tg截点值分别为167.59 ng/mL、116.38μg/L、21.74 mIU/L时,约登指数最大,诊断甲状腺癌的敏感度分别为78.57%、72.86%、68.57%,特异度分别为80.00%、92.85%、84.29%。甲状腺癌TNM分期为Ⅲ~Ⅳ期者血清Gal-1、Tg水平高于TNM分期为Ⅰ~Ⅱ期者(P<0.05),GPX3水平低于TNM分期为Ⅰ~Ⅱ期者(P<0.05);淋巴结转移者血清Gal-1、Tg水平高于未转移者(P<0.05),GPX3水平低于未转移者(P<0.05)。结论甲状腺癌患者血清Gal-1、GPX3、Tg水平与肿瘤分期及淋巴结转移具有相关性,检测Gal-1、GPX3、Tg水平有利于评估甲状腺癌疾病严重程度。Objective To analyze the relationship between levels of galectin-1(Gal-1),glutathione peroxidase 3(GPX3)and thyroglobulin(Tg)and disease severity of thyroid cancer.Methods 70 patients with thyroid cancer were selected,and 70 patients with benign thyroid lesions were selected as controls.The levels of serum Gal-1,GPX3 and Tg were detected in the 2 groups,and the levels of serum Gal-1,GPX3 and Tg were compared,and the diagnostic value of Gal-1,GPX3 and Tg on thyroid cancer was evaluated,and the relationship between clinical parameters and levels of Gal-1,GPX3 and Tg was analyzed among patients with thyroid cancer.Results The levels of serum Gal-1 and Tg in the thyroid cancer group were higher than those in the benign thyroid lesion group(P<0.05)while the GPX3 level was lower than that in the benign thyroid lesion group(P<0.05).When the cut-off values of serum Gal-1,GPX3 and Tg were 167.59 ng/mL,116.38μg/L and 21.74 mIU/L,the Youden index was the largest,and the sensitivities of diagnosing thyroid cancer were 78.57%,72.86%and 68.57%respectively,and the specificities were 80.00%,92.85%and 84.29%respectively.The levels of serum Gal-1 and Tg of thyroid cancer patients with TNM stageⅢ~Ⅳwere higher than those of patients with TNM stageⅠ~Ⅱ(P<0.05)while the level of GPX3 was lower than that of patients with TNM stageⅠ~Ⅱ(P<0.05).The levels of serum Gal-1 and Tg of patients with lymph node metastasis were higher than those of patients without metastasis(P<0.05)while the level of GPX3 was lower than those of patients without metastasis(P<0.05).Conclusion Levels of serum Gal-1,GPX3 and Tg in patients with thyroid cancer are correlated with tumor stages and lymph node metastasis.The detection of Gal-1,GPX3 and Tg is helpful to assess the severity of thyroid cancer.
