摘　　要：目的研究胸腹腔镜Ivor-Lewis食管癌切除术(MIILE)治疗食管中下段癌的临床疗效及安全性。方法选择86例食管中下段癌患者,按随机数字表法将其分为开放Ivor-Lewis食管癌切除术(OILE)组、MIILE组,各43例。MIILE组采用MIILE治疗,OILE组采用OILE治疗。比较两组围手术期指标、肺功能及并发症。结果MIILE组术中出血量少于OILE组,住院时间短于OILE组,术后第1 s用力呼气量占预计值百分比(FEV1%)、用力肺活量占预计值百分比(FVC%)高于OILE组,并发症总发生率低于OILE组,差异有统计学意义(P<0.05);两组淋巴结清扫数目、手术时间对比,差异无统计学意义(P>0.05)。结论食管中下段癌患者采用MIILE治疗损伤小、并发症少,可减轻对患者肺功能的影响,加快术后恢复,是治疗食管中下段癌的一种安全有效术式。Objective To study the clinical efficacy and safety of thoracic laparoscopic Ivor-Lewis esophageal cancer resection(MIILE)in the treatment of middle and lower esophageal cancer.Methods 86 patients with middle and lower esophageal cancer treated were selected and divided into open Ivor-Lewis esophageal cancer resection(OILE)group and MiILE group by random number table method,with 43 cases in each group.Miile group received Miile treatment,and OILE group received OILE treatment.Perioperative indexes,lung function and complications were compared.Results The intraoperative blood loss of MiILE group was less than that of OILE group,and hospital stay was shorter than that of OILE group,the percentage of forced expiratory volume in the first second after surgery to the predicted value(FEV1%)and percentage of forced vital capacity to the predicted value(FVC%)were higher than those in OILE group,and the total incidence of complications was lower than that in OILE group,the difference was statistically significant(P<0.05).There was no significant difference in the number of dissected lymph nodes and operation time between the 2 groups(P>0.05).Conclusion MIILE treatment has less damage and fewer complications for the treatment of middle and lower esophageal cancer,which can reduce the impact on the lung function of patients and accelerate postoperative recovery.It is a safe and effective procedure for the treatment of middle and lower esophageal cancer.
