摘 要:目的探讨动态增强磁共振(DCE-MRI)对肺部良恶性空洞性病变的诊断价值。方法选择肺部33例良性空洞性病变以及47例恶性病变患者的相关资料予以回顾性分析,分别作为良性组与恶性组,患者均行MRI平扫以及DCE-MRI扫描,分析其对肺部良恶性空洞病变的诊断价值。结果良性组与恶性组在空洞平均厚度以及直径,空洞内容物,空洞壁T1WI,空洞内部信号T1WI、T2WI、DWI、ADC信号强度以及TIC曲线等方面存在统计学意义(P<0.05),良性组与恶性组在空洞边界、外壁形态、空洞壁T2WI等方面差异比较无统计学意义(P>0.05);良性组与恶性组患者TTP、MAX_(Slope)、V_(e)、V_(p)、MAX_(Conc)比较无统计学意义(P>0.05),良性组患者K^(tarns)、K_(ep)明显低于恶性组患者(P<0.05);ROC曲线显示,K^(tarns)、K_(ep)等DCE-MRI半定量参数用于肺部良恶性空洞病变诊断AUC分别为0.820、0.785。结论肺良恶性空洞性病变应用DCE-MRI诊断价值优异,其可以从影像学表现以及定量参数方面有效鉴别肺良恶性空洞性病变。Objective To explore the value of dynamic contrast enhanced magnetic resonance imaging(DCE-MRI)in the diagnosis of benign and malignant lung cavity lesions.Methods The related data of 33 patients with benign lung cavity lesions(benign group)and 47 patients with malignant lesions(malignant group)were collected for retrospective analysis.All underwent MRI plain scan and DCE-MRI scan to analyze their diagnostic value for benign and malignant lung cavity lesions.Results The differences between benign group and malignant group in average thickness and diameter of cavity,contents in the cavity,T1WI of cavity wall,internal signals of cavity(T1WI,T2WI),DWI,signal intensity of ADC and TIC curve were statistically significant(P<0.05),while the difference in cavity boundary,morphology of outer wall or T2WI of cavity wall was not statistically significant(P>0.05).The difference between benign group and malignant group in TTP,MAX_(Slope),V_(e),V_(p)and MAX_(Conc) was not statistically significant(P>0.05).K^(tarns a)nd K_(ep)in benign group were significantly lower than those in malignant group(P<0.05).ROC curves showed that AUC values of DCE-MRI semi-quantitative parameters(K^(tarns),K_(ep))in the diagnosis of benign and malignant lung cavity lesions were 0.820 and 0.785,respectively.Conclusion The diagnostic value of DCE-MRI is great for benign and malignant lung cavity lesions.The imaging findings and quantitative parameters can be applied to effectively distinguish benign and malignant lung cavity lesions.
