摘 要:目的分析能谱CT成像联合血清癌胚抗原(CEA)、鳞状上皮细胞抗原(SCC)、细胞角蛋白19片段(CYFRA21-1)、胃泌素释放肽前体(proGRP)对肺腺癌与肺鳞癌的鉴别价值。方法选择肺腺癌与肺鳞癌患者各53例,共106例。使用能谱CT成像检查,检测血清SCC、NSE、CEA、proGRP、CYFRA21-1水平。比较2组能谱CT成像指标、血清SCC、NSE、CEA、proGRP、CYFRA21-1水平,分析能谱CT成像联合SCC、NSE、CEA、proGRP、CYFRA21-1的诊断价值。结果肺鳞癌钙含量、斜率K和NIC高于肺腺癌,差异比较均有统计学意义(P<0.05);而Effective-Z值与肺腺癌相近,差异比较无统计学意义(P<0.05)。肺腺癌患者血清NSE、CEACYFRA21-1、SCC、proGRP水平低于肺鳞癌患者(P<0.05)。能谱CT成像联合SCC、NSE、CEA、proGRP、CYFRA21-1鉴别肺腺癌与肺鳞癌价值最高,且高于以上任一或联二、联三、联四指标的鉴别价值(P<0.05)。结论能谱CT成像联合血清SCC、NSE、CEA、proGRP、CYFRA21-1能提升肺腺癌与肺鳞癌的鉴别价值,推荐使用。Objective To investigate the value of spectral CT imaging combined with serum carcinoembryonic antigen(CEA),squamous cell antigen(SCC),cytokeratin 19 fragment(CYFRA21-1)and pro-gastrin releasing peptide(ProGRP)in the differential diagnosis of lung adenocarcinoma and lung squamous cell carcinoma.Methods 106 patients were enrolled,including 53 cases of lung adenocarcinoma and 53 cases of lung squamous cell carcinoma.Spectral CT examinations and the test of serum levels of SCC,NSE,CEA,ProGRP and CYFRA21-1 were performed on all selected individuals,then the image parameters and serum levels of SCC,NSE,CEA,ProGRP and CYFRA21-1 were compared between groups,meantime,the diagnostic value of energy spectral CT imaging combined with SCC,NSE,CEA,ProGRP and CYFRA21-1 was evaluated.Results The lesion calcium content,slope of energy spectrum curve(K)and normalized iodine concentration(NIC)of patients with lung squamous cell carcinoma were higher than those of lung adenocarcinoma(P<0.05),while the Effective-Z value of two diseases yielded no significant difference(P>0.05).Serum levels of NSE,CYFRA21-1,SCC and proGRP in lung adenocarcinoma patients were lower than those in lung squamous cell carcinoma patients(all P<0.05).In differentiating lung adenocarcinoma from lung squamous cell carcinoma,energy spectral CT imaging combined with SCC,NSE,CEA,ProGRP and CYFRA21-1 achieved the highest value compared with separate,pairwise,triplet,or quadruplet examination(P<0.05).Conclusion Energy-spectral CT imaging combined with serum SCC,NSE,CEA,proGRP and CYFRA21-1 is of great clinical value in differentiating lung adenocarcinoma from lung squamous cell carcinoma,which is worthy of application.
