摘　　要：目的探讨甲胎蛋白异质体L3(AFP-L3)和高尔基体糖蛋白73(GP-73)在原发性肝癌(PLC)早期诊断中的应用及其与乙型肝炎病毒DNA(HBV-DNA)载量、肝功能的关系。方法选取94例PLC患者为PLC组,另取94例同期健康体检者为健康组。测定两组受试者AFP-L3和GP-73水平,PLC组HBV-DNA载量和肝功能指标[谷氨酸转氨酶(ALT)、天门冬氨酸氨基转移酶(AST)和总胆红素(TBIL)],比较两组AFP-L3和GP-73水平的表达差异,并统计分析AFP-L3和GP-73在PLC中的诊断效能及其与疾病进展的关系。结果PLC组AFP-L3和GP-73水平高于健康组(P<0.05);DNA高载量组AFP-L3和GP-73水平高于低载量组(P<0.05);DNA低载量组AFP-L3和GP-73水平高于阴性组(P<0.05);AFP-L3与ALT和AST呈正相关(P<0.05);GP-73与ALT、AST和TBIL均呈正相关(P<0.05);ROC曲线显示,AFP-L3诊断PLC的曲线下面积(AUC)为0.750,GP-73诊断PLC的AUC为0.899,两者联合诊断PLC的AUC为0.935,高于单独诊断(P<0.05)。结论血清AFP-L3和GP-73均可用于PLC早期诊断,且两者联合诊断价值更高,血清AFP-L3和GP-73与HBV-DNA载量和肝功能密切相关,可以用来评估疾病的进展。Objective To explore the application of alpha fetoprotein-L3(AFP-L3)and Golgi Protein 73(GP-73)in the early diagnosis of primary liver cancer(PLC)and their relationship with load of hepatitis B virus DNA(HBV-DNA)and liver function.Methods A total of 94 patients with PLC were enrolled as PLC group,while other 94 healthy controls during the same period were enrolled as healthy group.The levels of AFP-L3 and GP-73 in both groups,levels of HBV-DNA and liver function related indexes[alanine aminotransferase(ALT),aspartate aminotransferase(AST),total bilirubin(TBIL)]in PLC group were measured.The differences in expression levels of AFP-L3 and GP-73 between the two groups were compared.The diagnostic efficiency of AFP-L3 and GP-73 in PLC and their relationship with disease progression were statistically analyzed.Results The levels of AFP-L3 and GP-73 in PLC group were higher than those in healthy group(P<0.05),which were higher in high DNA load group than low DNA load group(P<0.05).The levels of AFP-L3 and GP-73 in low DNA load group were higher than those in negative group(P<0.05).AFP-L3 was positively correlated with ALT and AST(P<0.05),and GP-73 was positively correlated with ALT,AST and TBIL(P<0.05).ROC curves showed that the area under the curve(AUC)of AFP-L3 combined with GP-73 in the diagnosis of PLC was 0.935,greater than that of AFP-L3 and GP-73 alone(0.750,0.899)(P<0.05).Conclusion Both serum AFP-L3 and GP-73 can be applied in the early diagnosis of PLC,and diagnostic value of their combination is relatively higher.The serum AFP-L3 and GP-73 are closely related to HBV-DNA level and liver function,which can be applied to evaluate disease progression.
