高频超声引导下应用真空辅助活检系统对乳腺钙化灶行旋切活检的临床研究

摘　　要：目的探讨高频超声引导下应用真空辅助活检系统对乳腺钙化灶行旋切活检的临床研究。方法回顾性分析行乳腺活检的乳腺钙化灶患者62例107个病灶。记录107个病灶病理结果,美国放射学会的乳腺影像报告和数据系统(BI-RADS)分级与病理结果的关系、术后并发症。结果病灶病理结果良性病灶96例(89.7%),高危病灶7例(6.5%),恶性病灶4例(3.7%);乳腺超声BI-RADS分级Ⅱ级~ⅣC级良性病灶结果差异具有统计学意义(P<0.05);乳腺超声BI-RADS分级Ⅱ级~ⅣC级高危病灶结果差异具有统计学意义(P<0.05);乳腺超声BI-RADS分级Ⅱ级~ⅣC级恶性病灶结果差异具有统计学意义(P<0.05);62例患者术后出现局部血肿4例,乳头溢血2例,肿瘤残留2例,总并发症发生率为12.9%。结论高频超声引导下应用真空辅助旋切活检系统切除乳腺钙化灶,术后创伤小、并发症少,是辅助临床活检诊断乳腺疾病安全、有效的方法。Objective To investigate the clinical study of the application of vacuum-assisted biopsy system under the guidance of high-frequency ultrasound to perform rotary biopsy of breast calcifications.Methods Retrospectively reviewed 62 patients with breast calcified lesions who underwent breast biopsy,107 lesions.The pathological results,the relationship between the classification of the breast imaging report and data system(BI-RADS)of the American College of Radiology and the pathological results,and postoperative complications were recorded for 107 lesions.Results The pathological results of the lesions were 96 cases of benign lesions(89.7%),7 cases of high-risk lesions(6.5%),4 cases of malignant lesions(3.7%).The results of breast ultrasound BI-RADS grade II-IVC benign lesions showed statistical difference(P<0.05).The results of breast ultrasound BI-RADS gradingⅡ~ⅣC high-risk lesions were statistically significant(P<0.05);those of breast ultrasound BI-RADS gradeⅡ~ⅣC malignant lesions were statistically different(P<0.05).In 62 patients,there were local hematomas in 4 cases,nipple hemorrhage in 2 cases,and residual tumor in 2 cases.The total complication rate was 12.9%.Conclusions The application of vacuum-assisted rotary biopsy system under the guidance of high-frequency ultrasound to remove breast calcifications has less postoperative trauma and fewer complications.It is a safe and effective method to assist clinical biopsy in the diagnosis of breast diseases.
