机用镍钛器械与超声根管荡洗联合应用对有瘘型慢性根尖周炎的疗效

摘　　要：目的探究机用镍钛器械与超声根管荡洗联合应用于有瘘型慢性根尖周炎的临床疗效。方法选取2018年1月至2020年6月收治的有瘘型慢性根尖周炎患者90例,共90颗患牙。按照随机数字表法分为对照组和研究组,每组45例,45颗患牙。对照组患者采用机用镍钛器械预备根管,并使用注射器冲洗;研究组患者进行联合治疗,采用机用镍钛器械预备根管,并使用超声振荡冲洗。对2组患者封药后疼痛发生情况、瘘管消退情况进行观察和比较。结果研究组患者瘘管消退情况优于对照组患者,瘘管消退率37.78%,显著高于对照组患者(31.11%)(P<0.05);研究组患者术后1周疼痛情况明显优于对照组(P<0.05);研究组患者封药后1、3、7 d的疼痛情况均显著低于对照组(P<0.05)。结论应用机用镍钛器械与超声根管荡洗联合治疗有瘘型慢性根尖周炎有较好的应用效果,改善患者术后疼痛情况,有利于瘘管愈合,值得在临床中推广应用。Objective To investigate the clinical effects of nickel titanium instrument combined with ultrasonic root canal washing in treatment of chronic apical periodontitis with fistula.Methods A total of 90 patients with chronic apical periodontitis with fistula(90 teeth)who were treated in our hospital from January 2018 to June 2020 were enrolled in the study,who were randomly divided into control group and research group,with 45 cases(45 teeth)in each group.The patients in control group were treated by NiTi instruments to prepare root canal,and irrigation with syringe,however,those in research group were treated by NiTi instruments to prepare root canal,with ultrasonic oscillation.The occurrence of pain and fistula regression status were observed and compared between the two groups.Results The fistula regression rate in research group was significantly higher than that in control group(37.78%vs 31.11%,P<0.05).After 1 week treatment,the pain status in research group was significantly better than that in control group(P<0.05).Moreover the pain status at 1d,3d and 7d after drug sealing in research group was superior to that in control(P<0.05).Conclusion The NiTi instrument combined with ultrasonic root canal washing has obvious curative effects on chronic apical periodontitis with fistula,which can improve the postoperative pain status of patients,and is conducive to fistula healing,therefore,it is worthy of clinical application and promotion.
