调强适形放疗联合化疗对老年晚期宫颈癌患者生活质量、免疫功能以及疗效的影响

摘　　要：目的 分析调强适形放疗与化疗联合治疗老年晚期宫颈癌的临床效果。方法 选取我院2016年1月至2018年1月收治的80例老年晚期宫颈癌患者为研究对象,采用随机数字表法将其分为对照组和研究组,各40例。对照组接受调强适形放疗,研究组接受调强适形放疗联合化疗,比较两组的治疗效果。结果 治疗后,两组的白细胞分化抗原3阳性(CD3^(+))、白细胞分化抗原4阳性(CD4^(+))、CD4^(+)/白细胞分化抗原8阳性(CD8^(+))均低于治疗前,但研究组高于对照组,差异具有统计学意义(P<0.05)。治疗后,两组的糖类抗原125(CA125)、鳞状上皮细胞癌抗原(SCCA)、癌胚抗原(CEA)水平均低于治疗前,且研究组低于对照组,差异具有统计学意义(P<0.05)。治疗后,两组患者的生活质量各维度评分均高于治疗前,且研究组高于对照组,差异具有统计学意义(P<0.05)。研究组的近期治疗总有效率为72.50%,高于对照组的50.00%,差异具有统计学意义(P<0.05)。研究组的2、3年生存率均显著高于对照组,远处转移率、局部复发率低于对照组,差异具有统计学意义(P<0.05)。两组的毒副反应总发生率比较,差异不显著(P>0.05)。结论 调强适形放疗联合化疗治疗老年晚期宫颈癌的近远期疗效确切,能够有效降低肿瘤标志物水平,改善患者的生活质量,减少疾病复发或转移,且对免疫功能影响小,值得推广。Objective To analyze the clinical effect of intensity-modulated conformal radiotherapy combined with chemotherapy in the treatment of elderly patients with advanced cervical cancer.Methods A total of 80 elderly patients with advanced cervical cancer admitted in our hospital from January 2016 to January 2018 were selected as the research objects and divided into control group and study group by random number table method,with 40 cases in each group.The control group received intensity-modulated conformal radiotherapy,and the study group received intensity-modulated conformal radiotherapy combined with chemotherapy.The therapeutic effects of the two groups were compared.Results After treatment,the cluster of differentiation 3 positive(CD3^(+)),cluster of differentiation 4 positive(CD4^(+)) and CD4^(+)/cluster of differentiation 8 positive(CD8^(+)) in both groups were lower than those before treatment,but those in the study group were higher than the control group,the differences were statistically significant(P 0.05).Conclusion Intensity-modulated conformal radiotherapy combined with chemotherapy in the treatment of elderly patients with advanced cervical cancer has a definite short-term and long-term efficacy,it can effectively reduce the levels of tumor markers,improve the quality of life,reduce the disease recurrence or metastasis,with little impact on immune function,which is worthy of promotion.
