摘  要:目的探讨Quantra软件与乳腺影像报告和数据系统(BI-RADS)分型2种方法评估女性乳腺X线密度的一致性及二者的相关性。方法回顾性分析2019年7月至2021年6月间山西省人民医院放射科无症状女性参加乳腺健康体检BI-RADS 1~2类者1780例,使用Quantra软件与BI-RADS分型法评估乳腺X线密度。结果Quantra分型与2位医师BI-RADS分型间具有较好的一致性(Kappa=0.619、Kappa=0.646)并具有较强的正相关性,2位医师BI-RADS分型间具有很好的一致性(Kappa=0.870),不同BI-RADS分型间乳腺乳腺纤维腺体体积(VFG)、乳房体积(VB)及乳腺纤维腺体体积(VBD)百分比间秩和检验差异均有统计学意义(P<0.001),随着BI-RADS分型的增高,乳腺VFG及VBD百分比均增加。结论Quantra分型和BI-RADS分型2种方法间有较好地一致性及明显的正相关性;随着BI-RADS分型的增高,乳腺VFG及VBD百分比均增加。Objective To evaluate the consistency in breast density on mammography as assessed by Quantra breast volumetric density software vs breast Imaging reporting and data system(BI-RADS) classification and the correlation between the both approaches. Methods Included in this study were 1 780 asymptomatic women who were referred to Department of Radiology in Shanxi Provincial People′s Hospital for breast checkup between July 2019 and June 2020 and were classified with BI-RADS categories 1-2. The breast density on mammography was further evaluated by Quantra and BI-RADS classification. Results There was a good consistency(Kappa =0.619, and Kappa =0.646) with strong positive correlation between quantized area breast density by Quantra and BI-RADS classification by the two physicians. There was also a good correlation in BI-RADS classification between the two physicians(Kappa =0.870). There were significant differences in the proportions of fibroglandular tissue volume(VFG),total breat volume(VB)and volumetric breast density(VBD)across BI-RADS classifications by rank sum test(all P <0.001). The proportions of VFG and VBD increased along with higher BI-RADS classifications. Conclusion There is a good consistency and significantly positive correlation between the Quantra and BI-RADS classification. Higher BI-RADS classification is associated with greater percentages of breast VFG and VBD.
