布地奈德混悬液对胸腔镜微创食管癌患者肺功能保护临床研究

摘　　要：目的:研究布地奈德混悬液对腔镜微创食道癌手术患者肺功能的保护作用。方法:选取2017年3月至2019年3月在赣南医学院第一附属医院胸心外科就诊行腔镜微创食道癌手术患者100例为研究对象,按是否使用布地奈德混悬液治疗分为观察组和对照组,观察组(布地奈德混悬液组)50例,对照组(无布地奈德混悬液组)50例,比较两组患者的术后肺功能[第1秒肺活量占整个肺活量百分比(FEV1%)、每分钟最大通气量实测值与理论预计值百分比(MVV%)]和肺部并发症(肺部感染、呼吸功能不全、肺不张)发生情况。结果:术前两组患者肺功能差异无统计学意义(P>0.05);术后观察组FEV1%、MVV%高于对照组,差异有统计学意义(P<0.05);术后观察组肺部感染、呼吸功能不全、肺不张发生率低于对照组,但差异无统计学意义(P>0.05)。结论:微创腔镜食管癌患者使用布地奈德混悬液后对术后肺功能有保护作用,有效减少患者术后肺部并发症。Objective:To study the protective effect of budesonide suspension on lung function in patients undergoing minimally invasive endoscopic esophageal cancer surgery.Methods:The medical data of 100 patients who underwent endoscopic minimally invasive surgery for esophageal cancer from March 2017 to March 2019 in the Department of Cardiothoracic Surgery,The First Affiliated Hospital of Gannan Medical University were analyzed.They were randomly divided into experimental group(50 cases) of budesonide suspension group and control group(50 cases) of nonbudesonide suspension group.The indexes of postoperative pulmonary function and pulmonary complications were compared between the two groups:FEV1%,MVV%,pulmonary infection,respiratory insufficiency and incidence of atelectasis.Results:There was no significant difference in pulmonary function between the two groups(P>0.05)before operation.From the first second after operation,the percentage of vital capacity in the whole vital capacity(FEV1%),the percentage of the measured value of maximum ventilation per minute and the theoretical predicted value(MVV%)in the experimental group was higher than that in the control group(P0.05).Conclusion:The use of budesonide suspension in patients with minimally invasive endoscopic esophageal cancer has a protective effect on postoperative pulmonary function and effectively reduces postoperative pulmonary complications.
